7/28/2014 Please conduct a site visit to determine if there are any obvious sanitary defects that could be contributing to the ACUTE coliform bacteria MCL violations. This is the PWS's 2nd ACUTE bacteria MCL violation in less than 12 months; therefore, well evaluations and continuous disinfection or a new water source will be required. Please also identify any items that may require correction at the PWS; and might therefore impact a compliance schedule if we determine continuous disinfection is required (i.e., buried pressure tanks, well in a pit, inadequate well casing, well construction not to standards, structure that could be used to house equipment, etc.). If possible, photos of the wells and pressure tank(s) would be helpful. Thanks. MARIE LEAT

8/1/2014 Work request investigation conducted on 7/31/14. Observed some problems that could contribute to bacterial contamination: cross connection between potable water supply and the watering system in the chicken barns, potable water lines interconnected with production water lines after the RPZ in the production building, and no inspection tag on the RPZ. There are also some potential problems that I could not directly observe: possible leaky foot valves on the submersible well pumps, possible damage to the water tower vent screen, and there may be other production water lines not adequately separated from the potable water.
