11/6/2025 Lee Trask with Van Diest Supply Co called to report the anhydrous ammonia release. 1500 gal was released from a double nurse tank. Wind was 8 mph out of the South (Madrid is due north ~2 miles). Ammonia Fog was 1.5 miles from tank release. No one could verify field tiles, or creeks in the immediate area. Approximately a mile to the west is the Des Moines River...so hopefully ammonia dispersed and didn't sink towards the water. An NRC was filed after my call with Mr. Trask (phone 515-450-1790). I'll email Lee Trask with the 30 day HSI report. JEFF THEOBALD
11/6/2025 On 11/5/2025 at 10AM Ted Peterson spoke with Chris Hayes. The two tanks were located in the middle of the field, no water sources nearby. A shelter in place directive was authorized for some residents nearby out of an abundance of caution. He confirmed nurse tanks emptied to the atmosphere; by the time HazMat got to the site. No water suppression was used by FD or Hazmat. JEFF THEOBALD
