11/28/2023 11/28/2023: Received the attached Iowa DNR UST Section Equipment Form. JENIFER HARKIN
10/9/2023 Received the following information from MercyOne: "Below is our timeline and amount of pipe we replaced. Hope this is helpful. On 8/14/23 our licensed installer drove to job site and met with Mark Shirly's (Seneca’s Testing Group) guys. They performed a tightness test on the tank and lines to make sure everything was good underground once we capped the broken pipes, and everything passed. We started uncovering vent piping and remote fill where it was broken, pipes ran under the concrete, guys were going to have to remove more concrete. We also found the remote fill went through a chase way, through a wall over to the filter. With finding this we wanted to make sure nothing more was damaged, so we had the testers come back the following day to check it. We also contacted Hazardous Waste to come out to remove water out of tank. On 8/15/23, we met with Kenny from Hazardous Waste, he pumped out water from the tank. The testers tested the remote fill going through the wall back to the filter. It was ok, we dug up what soil we could and put it in plastic due to the smell and had SES (Seneca Environmental Services) come out to take samples to be tested. We then repaired approximately 20’ of fiberglass vent and fill pipe. We then had the testers come back and test entire system. All passed tightness and soap testing. On 8/17, we met with the transport driver, he dumped fuel in the tank while checking out the remote fill with no visual signs of leaks. Also, DNR stopped by and talked to Jacob and she told him everything looked fine. We picked up a 55 gal. drum and filled it with the soil we dug up and was kept at our office until soil sampling results came back. Got a load of pea gravel and back filled trench to cover up piping system. Soil removed was then disposed of per our environmental and waste solutions departments.
