			Recommended Standards for Water Works 3.2.7 requires that wells be sealed properly to prevent the entrance of contamination. The electrical conduit that serves the well is broken. The conduit must be replaced or repaired.
			Subparagraph 567 IAC 81.2(4) requires that an Affidavit form provided by the director and shall be completed and include the name and signature of the certified operator, the operator’s certification number, class and grade, and the date of last renewal of the operator’s certificate. Four duly notarized copies of the affidavit must be returned to and approved by the director. Submit the attached Operator by Affidavit forms within 45 days of receipt of this report.
