In the lower half of the page you should put a number in those small boxes on the left. For example, a 1 for how many turbidimeters are in use and the same for how many were verified weekly and a 0 if none were out of specifications. Be sure you are keeping a separate log of the verifications performed or you can use the table provided on the form. Your name was not included on the MOR. Please include it and print and sign the monthly reports for your on-site records. You don't need to submit any revisions this month, but do things differently on the May 2019 MOR calibration/verification page. And please let me know if you need anything during this transition.
