Member Login



Description:
Provides access into the secure area of the Private Well Tracking System system. User must have a valid login id and password.
 
Fields:
Username: (Required) The username of the individual logging into the member's area. To request a username or obtain assistance with forgotten passwords, contant Brent Parker at the DNR.
Password: (Required) Secure password for the username entered above. A minimum of five characters must be entered.
Remember me next time: If this box is checked when you log in, then you will not need to log in again next time.
 
Buttons:
Log In: Attempts to log the person into the system. If successful, the user will be returned to the menu with a new menu list provided access to the secure portions of the system.

Forgot Your Password?: Takes the user to the Password Retrieval screen.

Password Retrieval



Description:
Resets a user's password to a random set of characters and e-mails it to them. After logging in with the new password the user can go to the Change Password page in the member's area.
 
Fields:
Username: (Required) The username of the individual who needs their password reset and sent to them. An e-mail address must have been provided for the user.
 
Buttons:
Submit: Attempts to send the new password to the user. A confirmation or an error message are displayed.

Change Your Password



Description:
Provides a method for the user to change their password while logged into the system.
 
Fields:
Password: (Required) The user's current password that was used to log into the system. A minimum of five characters must be entered.
New Password: (Required) The new password that the user would like to change to. A minimum of five characters must be entered.
Confirm New Password: (Required) The new password that the user would like to change to (re-type to confirm spelling). A minimum of five characters must be entered.
 
Buttons:
Change Password: Submits the new password to the system. If all of the information is valid, then the user will be taken to the home page. If not, an error message will be displayed.
Cancel: Cancels out of the password change process and returns the user to the home page.

Permit Search



Description:
Search for an existing permit. The user can get the permit information by entering the appropriate search criteria, which includes permit details, property owner information and well location details and clicking on the search button.
 
Fields:
Permit Number: The unique number generated by the system for identifying each permit.
Permit Date: The date that the permit was issued.
Certified Well Driller: The name and current certification number of the well driller who is likely to drill the well(s) listed in the permit.
Expired Well Driller: The name and expired certification number of the well driller who was likely to drill the well(s) listed in the permit. This applies if the well driller had a certification number that is now expired at the time when the permit was issued.
County: The name of the county where the well(s) listed in the permit are located.
First Name: The property owner’s first name.
Middle Name: The property owner’s middle name.
Last Name: The property owner’s last name.
Company: The name of the company.
Quarter 5: The quarter of the quarter of the quarter of the quarter of the quarter of the section, township, range of the well.
Quarter 4: The quarter of the quarter of the quarter of the quarter of the section, township, range of the well.
Quarter 3: The quarter of the quarter of the quarter of the section, township, range of the well.
Quarter 2: The quarter of the quarter of the section, township, range of the well.
Quarter 1: The quarter of the section, township, range of the well.
Section: The section of the well location.
Township: The township of the well location.
Range: The range of the well location.
Direction: The direction of the well location.
Parcel Number: The value used at the county level for identifying a well location.
E911 House Number: The house number of the E911 address of the well.
E911 Street Address: The street address of the E911 address of the well.
 
Buttons:
Submit: Takes the user to the Permit Overview screen, which displays a list of header information for the permits that match the search criteria.
Reset: Resets all of the fields to their default value.

Permit Overview



Description:
Displays a list of permits matching the search criteria entered by the user in the Permit Search screen. The user can get the details of a particular permit by clicking on the permit number.
 
Fields:
Permit Number: The unique number generated by the system for identifying each permit. Clicking on the permit number takes the user to the permit's details.
Permit Date: The date that the permit was issued.
Property Owner: The name of the person or company that owns the property.
Status: The status of a permit, which can be Active, Completed, Log Completed, Water Test Completed, Expired or Withdrawn.
County: The name of the county where the well(s) listed in the permit are located.
Certified Well Driller: The name and current certification number of the well driller who is likely to drill the well(s) listed in the permit.
Expired Well Driller: The name and expired certification number of the well driller who was likely to drill the well(s) listed in the permit. This applies if the well driller had a certification number that is now expired at the time when the permit was issued.
Well Location: The PLS coordinates of the well location (QQQQQSTR).
 
Buttons:
Return: Takes the user back to the Permit Search screen.

Permit Details



Description:
This purpose of this screen is to maintain permit information. The user can create, view, update or delete permit information depending on their privileges.
 
Fields:
County Name: (Required) The name of the county where the well(s) listed in the permit are located.
Well Purpose: (Required) The purpose of the well(s) in the permit.
Permit Number: The unique number generated by the system for identifying each permit.
County Agent: (Required) The name of the county agent issuing the permit.
Permit Date: (Required) The date that the permit was issued.
Contract Company: The name of the contractor company the well driller is associated with. Along with the contractor company name, the city in which it resides is also displayed.
Well Driller: (Conditional) The name and current certification number of the well driller who is likely to drill the well(s) listed in the permit. This is required if the expired well driller drop-down is not displayed or does not have a value selected.
Exp. Driller: (Conditional) The name and expired certification number of the well driller who was likely to drill the well(s) listed in the permit. This applies if the well driller had a certification number that is now expired at the time when the permit was issued. This only displays if it has a value, and is required if a certified well driller is not selected.
Role: (Required) The applicant’s role which can be either Certified Driller, Owner or Other.
Applicant First Name: (Conditional) The first name of the applicant. If a company name is not entered then the first or middle name is required.
Applicant Middle Name: (Conditional) The middle name of the applicant. If a company name is not entered then the first or middle name is required.
Applicant Last Name: (Conditional) The last name of the applicant. If a company name is not entered then the last name is required.
Applicant Company: (Conditional) The company that applied for the permit. If a first or middle and last name are not entered then the company is required.
Applicant Street: (Required) The applicant's street address.
Applicant City: (Required) The applicant's city.
Applicant State: (Required) The applicant's state.
Applicant Zip: (Required) The applicant's zip code.
Applicant Phone: (Required)The applicant's phone number.
Property Owner First Name: (Conditional) The first name of the property owner. If the applicant is not the owner and a company name is not entered then the first or middle name is required.
Property Owner Middle Name: (Conditional) The middle name of the property owner. If the applicant is not the owner and a company name is not entered then the first or middle name is required.
Property Owner Last Name: (Conditional) The last name of the property owner. If the applicant is not the owner and a company name is not entered then the last name is required.
Property Owner Company: (Conditional) The company that owns the well. If the applicant is not the owner and a first or middle and last name are not entered then the company is required.
Property Owner Street: The property owner's street address.
Property Owner City: The property owner's city.
Property Owner State: The property owner's state.
Property Owner Zip: The property owner's zip code.
Property Owner Phone: The property owner's phone number.
Well Number: The unique number generated by the system for identifying each well. Clicking on the well number takes the user to the well's details.
Permit Well Status: The status of the permit for a well, which can be Active, Completed, Log Completed, Water Test Completed, Expired or Withdrawn.
E911 House Number: The house number of the E911 address of the well.
E911 Street: The street address of the E911 address of the well.
E911 City: The city of the E911 address of the well.
Parcel Number: The value used at the county level for identifying a well location.
Construction Date: (Required) The anticipated construction date of the proposed well.
Depth: (Required) The estimated depth of the proposed well.
Map Type: (Required) The method in which the latitude and longitude coordinates were found.
Latitude: (Required) The user can either enter the latitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
Longitude: (Required) The user can either enter the longitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
PLS: This information is read-only. This information will automatically be populated when the user returns from the Map screen. If PLS information is based on estimated section-corners, “(estimated)” will appear after the PLS description.
Purpose: (Required) The use(s) of the well.
Variance Requested: Any variances that have been approved for the permit.
Special Conditions: Any special conditions that have been approved for the permit.
 
Buttons:
Search Wells: Only displays when adding an existing well. Searches for a well log matching the well number entered into the text box. If the well number is found then the well information is displayed. If it is not currently tied to a permit and is the same type as the current permit (Heat Pump, Monitoring Well or Water Supply Well and Other Uses), then the "Add Well to Permit" link will be displayed, otherwise an error message will be displayed.
Cancel: Only displays when adding an existing well. Cancels the adding of an existing well.
Get Map...: Opens the map window. In the map window the user can navigate to the location of the proposed well and capture the center point of the map to get the latitude and longitude.
Save: Saves the permit. If the user does not have permission to edit the permit then the button will be disabled.
Cancel: Returns the user to the Permit Search screen.
Delete: Deletes the permit. This only displays when editing an existing permit. If the user does not have permission to delete the permit then the button will be disabled.
 
Links:
Add New Well: Adds a new set of blank well fields to the permit. After the required fields are filled in and the permit is saved the proposed well will be added to the permit.
Add Existing Well: Displays a text box for entering a well number, a search button for searching for the well number and a cancel button.
Add Well to Permit: Ties an existing well to the permit. This only displays when adding an existing well.
Well Number: Takes the user to the well's details.
Update Status: Updates the permit status for a proposed well. This only displays when the user is a sanitarian that does not have permission to edit the rest of the permit.

Well Log Search



Description:
Search for an existing well or heat pump log. The user can get the well or heat pump log information by entering the appropriate search criteria, and clicking on the search button.
 
Fields:
Certified Well Driller: The name and current certification number of the well driller who drilled the well or heat pump.
Expired Well Driller: The name and expired certification number of the well driller who drilled the well or heat pump. This applies if the well driller had a certification number that is now expired at the time when the well was drilled.
Well Number: The unique number generated by the system for identifying each well or heat pump.
Drill Date From: The well or heat pump drill completion date (beginning date range).
Drill Date To: The well or heat pump drill completion date (ending date range).
County: The name of the county where the well or heat pump is located.
Geosam Import: Wells that were transferred from the Iowa Geological Survey.
First Name: The property owner’s first name.
Middle Name: The property owner’s middle name.
Last Name: The property owner’s last name.
Company: The name of the company that owns the property.
First Name: The well user’s first name.
Middle Name: The well user’s middle name.
Last Name: The well user’s last name.
Company: The name of the company that uses the well.
Quarter 5: The quarter of the quarter of the quarter of the quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 4: The quarter of the quarter of the quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 3: The quarter of the quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 2: The quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 1: The quarter of the section, township, range of the well or heat pump.
Section: The section of the well or heat pump location.
Township: The township of the well or heat pump location.
Range: The range of the well or heat pump location.
Direction: The direction of the well or heat pump location.
Parcel Number: The value used at the county level for identifying a well or heat pump location.
E911 House Number: The house number of the E911 address of the well or heat pump.

Well Log Overview



Description:
Displays a list of wells or heat pumps matching the search criteria entered by the user in the Well Log Search screen. The user can get the details of a particular well by clicking on the well number.

Well Number: The unique number generated by the system for identifying each well or heat pump. Clicking on the well number takes the user to the well or heat pump's details.
County: The name of the county where the well or heat pump is located.
Property Owner: The name of the person or company that owns the property.
Certified Driller: The name and current certification number of the well driller who drilled the well or heat pump.
Expired Driller: The name and expired certification number of the well driller who drilled the well or heat pump. This applies if the well driller had a certification number that is now expired at the time when the well or heat pump was drilled.

Well Details



Description:

County: (Required) The name of the county where the well is located.

Geosam Reviewed: Indicates if the Iowa Geological Survey has reviewed the well record.
Property Owner First Name: (Conditional) The first name of the property owner. If a company name is not entered then the first or middle name is required.
Property Owner Middle Name: (Conditional) The middle name of the property owner. If a company name is not entered then the first or middle name is required.
Property Owner Last Name: (Conditional) The last name of the property owner. If a company name is not entered then the last name is required.
Property Owner Company: (Conditional) The company that owns the well. If a first or middle and last name are not entered then the company is required.
Property Owner Street: (Required) The property owner's street address.
Property Owner City: (Required) The property owner's city.
Property Owner State: (Required) The property owner's state.
Property Owner Zip: (Required) The property owner's zip code.
Property Owner Phone: The property owner's phone number.
Map Type: (Required) The method in which the latitude and longitude coordinates were found.
Latitude: (Required) The user can either enter the latitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
Longitude: (Required) The user can either enter the longitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
PLS: This information is read-only. This information will automatically be populated when the user returns from the Map screen. If PLS information is based on estimated section-corners, “(estimated)” will appear after the PLS description.

Date Completed: (Required) The date that well drilling was completed.
Date Completed Confidence: (Required) The confidence level that the date completed is accurate.
Samples have been submitted to IGS for analysis: Indicates whether or not water samples have been submitted to the Iowa Geological Survey for analysis.
Well Depth: (Required) The depth of the well.
Well Depth Confidence: (Required) The confidence level that the well depth is accurate.
Depth to Water: The depth to the surface of the water.
Topography: The topographical position of the well on the landscape.
Elevation: The relative elevation, compared to sea level, of the wellhead.
Construction Method: (Required) The type of well construction equipment used to construct the well.
Data Entered By: The name of the person entering the well details (defaults to the current user's username).
Well Driller: (Required) The name of the well driller who drilled the well. Along with the well driller’s name, the certification number is also displayed. If the well driller had a certification that is now expired an additional drop-down will be displayed for expired well drillers.
Contract Company: The name of the contractor company the well driller is associated with. Along with the contractor company name, the city in which it resides is also displayed.
Well Use: (Required) The use(s) of the well.
Descriptor: (Conditional) The value used to identify a monitoring well. This is only required for monitoring wells.
Number of Wells: (Conditional) The number of heat pumps drilled under the associated permit. This is only required for heat pumps.
Well Was: (Required) The method used to develop the well. This field is not required when editing older well logs.
For: (Required) The length of time the well was pumped or developed. This field is not required when editing older well logs.
At: (Required) The pumping rate used to develop the well in gallons per minute. This field is not required when editing older well logs.
Explain: Further explaination of the well development.
Pump Installed: Indicates whether or not the pump was installed.
Date: (Conditional) The date that the pump was installed. This is required if the pump has been installed.
Type of Pump: (Conditional) The type of pump installed. This is required if the pump has been installed.
Pump Diameter: (Conditional) The diameter of the pump installed. This is required if the pump has been installed.
Installer's Name (text box): The name of the pump installer who installed the pump. This field is for older well logs which had the pump installer's name typed in.
Installer's Name (drop-down): (Conditional) The name of the pump installer who installed the pump. Along with the pump installer’s name, the certification number is also displayed. This is required if the pump has been installed.
Depth to Intake: (Conditional) The depth from the ground surface to the level of the pump intake. This is required if the pump has been installed.
Rated Capacity: (Conditional) The capacity of the well pump in gallons per minute. This is required if the pump has been installed.
From: (Required) The depth of the top of this segment of the well.
To: (Required) The depth of the bottom of this segment of the well.
Lithology: (Required) A description of the material in this segment of the subsurface formation.
Color: (Required) The color of the soil.
Comments: Any other useful information.
Remarks: Such as depth of lost circulation, materials or tools.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Hole Size: (Required) The diameter of the hole constructed in the ground.
Comments: Any other useful information.
Casing Type: (Required) The material used to construct the casing for this segment of the hole.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Diameter: (Required) The diameter of the casing for this segment of the hole.
ID/OD: (Required) Indicates whether the diameter specified is the inside or outside diameter.
STD/Thickness: (Required) The casing weight per foot for steel casing or wall thickness for plastic casing.
Comments: Any other useful information.
Grouting Type: (Required) The grout material used.
Grouting Method: (Required) The method used for grouting.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Amount: (Required) The amount of grout used.
Amount Unit: The measurement unit of the amount of grout.
Comments: Any other useful information.
Material: (Required) The well screen material such as stainless steel or plastic.
Diameter: (Required) The diameter of the well screen.
Slot Size: (Required) The well screen slot size in thousandths of an inch.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Comments: Any other useful information.
Bottom Capped: Indicates whether or not a cap has been placed on the bottom of the casing.
Capping Material: (Conditional) The type of bottom cap that was used. This is required if a cap has been placed on the bottom of the casing.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Gravel Type: (Required) The material used for gravel pack.
Weight: (Required) The weight of the gravel pack in pounds.
Comments: Any other useful information.
Seals Type: The type of seal used.
Seals Depth: The depth that the seal was placed.
Packers Type: The type of packer used.
Packers Depth: The depth that the packer was placed.
Aquifer: (Required) The type of material that constitutes the primary water bearing aquifer of this well. This field is not required when editing older well logs.
Main Water-Supply Zone from: (Required) The depth of the top of the primary aquifer supplying this well. This field is not required when editing older well logs.
To: (Required) The depth of the bottom of the primary aquifer supplying this well. This field is not required when editing older well logs.
