10/21/2024 I spoke to a UST Professional that has been contracted to conduct the monthly walkthrough inspections for this facility. The UST Professional reported potential concerns that they have with the installation of the 15,000 gallon jet fuel UST that they have observed while onsite. The 15,000 gallon jet fuel UST was installed on May 1, 2024. I have attached photographs taken by the UST Professional during the August walkthrough inspection and during the October walkthrough inspection. The UST Professional expressed concerns with not only the cracking of the concrete over the UST, but also the widening of the crack just within the last few months. The UST Professional is concerned that the tank may not have been properly installed and could be floating. It was reported that the facility continues to pass all of its leak detection tests. But if this is indeed what is occurring, there could be issues down the road. Lastly, it was reported to me that the old UST at this location contained a remote fill. It was reported that the remote overfill was not removed during the new tank install, but is also no longer used. Could the field office visit the facility to provide any input as to whether or not it appears the tank is floating/rising? Could the field office inspect the former remote fill and determine if this could be a source of groundwater/surface water to enter the tank basin and cause it to float? Could the field office inspect the former remote fill and ensure it is at least locked so a delivery driver cannot unload fuel into it? Can the field office review all leak detection records to ensure everything continues to pass? And anything else the field office determines necessary. BILL GROSS

10/25/2024 Re: 198606180 Iowa Methodist Medical Center UST Work Request Gastineau, Janet <janet.gastineau@dnr.iowa.gov> 10:05?AM (12 minutes ago) to Bill, Keith, Ted, Brian I was on site yesterday. Facility operations is well aware of the concrete problem and is working with Acterra to get the concrete replaced. The individual that performs the 30-day walkthrough inspections is a hospital employee and has kept facility operations informed of the changes. Acterra reported soil was added over the rock backfill prior to laying the concrete, which caused the cracking/shifting. The soil will be removed, additional rock placed and the slab will be repoured. The remote fill from the former UST at this location is approximately 60 feet to the east and does not have a cap on it. They believe it was capped/cut off but don't know where. I sent a message into Acterra about as-builts in this regard and strongly urged facility operations staff to put a locked cap on it. The pilots, which are contract employees, manage the fuel deliveries and I was assured they know where the fuel is delivered for the new UST. I will be developing a routine audit report. I've asked the facility to put a locked cap on it to ensure there isn't an errant fill. I was on site yesterday and they also let me know your group has plans to replace the concrete over the tank.
