7/24/2009	11/13/03 1300 hrs. Dennis Appelhons traveled to Eddyville to review this spill incident with Cargill officials. Interviews were conducted with Mr. Dave Olson, Environ. Coordinator, Ms. Angie Chiles, Project Engineer, Mr. Milo Aljet, Process Engineer. Dave Olson reported the spill incident per IDNR requirements. However, the state radio dispatcher mistakenly paged F.O. 6, believing that all of Eddyville was in Wapello Co. instead of Monroe Co. F.O. 6 staff instructed Mr. Olson to contact F.O. 5 phone number. Due to the Veterans Day office closure, Mr. Olson received our usual after hours telephone recording, which lists our office staff business phone numbers. Instead of calling the 24 hour spill number again, Mr. Olson left a phone mail message on Jim Stricker's office phone and also on bill Gross's cell phone. Both Jim and Bill reviewed their phone messages the next workday (11/12/03) and informed Dennis Appelhons of the phone messages. I informed Dave Olson that in the future when contacting state radio communications about a spill, inform them that they must activate F.O. 5 pager. Dave indicated that he would also put that note in the spill response plan for the facility to ensure all after hours shift supervisors were aware of this fact. Dave will forward a written spill report within 30 days per Chapter 131.2(2) requirements.	DNRHSIADMIN
