8/28/2012	On August 28, 2012 this office conducted a routine site visit at the above appliance demanufacturing facility. This office had received questions on the 2011 annual report submitted by the facility. In August of 2011 this facility began a new record keeping system as the previous system was inadequate. While on site, I reviewed records from the most recent shipment of PCB capacitors and mercury switches. Shipments were made in May of 2012. I reviewed daily logs and observed that no PCB capacitors were accounted for. This facility is also removing mercury vapor lamps for cities. It was unclear whether all of the PCB's were coming from mercury lamps or inaccurate daily logs were being kept. I looked through the capacitor/ballast non PCB container and found no improperly placed components. I will follow up with the site on this issue. Appliances were being stored well as were mercury switches and fluorescent tubes. A Line plans on expanding their demanufacturing operations. They also noted that they no longer accept appliances that have components removed or have been cut prior to arrival.	AMBER SAUSER
