UST inspection Mr. Bennett, I conducted a follow up visit to this facility yesterday and there were no records on site. Mr. Lowe was helpful, but there is additional information I need before I can complete my report. Department rule 567 IAC 135.4(5)”c” requires owners and operators to keep records at either the UST site and immediately available for inspection or at a readily available alternative site and be provided for inspection to the department upon request. The department’s interpretation of the above rule is that records may be maintained at an off site location; however, the owner/operator must be able to produce the records and make them available within two business days of the request, excluding the day of the request. Therefore, please forward the following records to this office within two business days or by Tuesday, May 29, 2012: • The filter was observed to be leaking (see photo) at the time of my photo but Mr. Lowe said he would correct the same day. Please send documentation that this was complete. • Log of the interstitial monitoring conducted for the past 12 months. • Line tightness test and automatic line leak detector test results conducted in the past 12 months. • Records demonstrating corrosion protection standards are met for that portion of the UST system constructed of steel. Please submit the results of the most recent test of the cathodic protection system. • Documentation of A/B operator certification. I did not see emergency procedures or contact information posted at the dispenser area (with the exception of a sign–attached photo); this is a requirement of the new operator training rules and improvements must be made.
