12/4/2023	Visited at request of Cindy Garza to determine compliance with construction permit. Permit allowed for construction of new barn and reception pit. Instead the facility installed an outside flush tank and began using the barn and flush tank without final approval from permit writer. While onsite I gathered details on the changes and verbally notified Brian Sipley that he will be receiving a Notice of Violation letter from me.	BRIAN JERGENSON

			Emergency response plan received 5/16/24 and was acceptable. Secondary containment structure is installed and signage is in place as of submittal 5/29/24 and penalty has been paid. All requirements in ACO have been met as of 5/29/24.
