7/15/2021 On 07/15/2021 at ~0844 Aaron Pickens forwarded an email that Sleepy Hollow Campground was under new management. Per the sanitary survey on 05/06/2021, the following deficiencies were noted: Summary of Minor Deficiencies - 1. Develop a lead and copper sampling plan indicating where tap water samples are to be taken and the correct number to be taken during each monitoring period. 2. Ensure chlorine residuals are monitored and recorded daily for both distribution systems (950 & 951), as outlined in the operation permit, and that the samples are rotated among different locations throughout the distribution system. [567 IAC 42.4(3)"a"(2)5 and 43.6(1)"c"(2)1]. Summary of Requirements - 1. Complete and submit the enclosed Operation & Maintenance Plan by June 30, 2021. [567 IAC 81.16(455B)] Per a discussion with Aaron Pickens, it was determined that due to a change in owners, the deficiencies would be closed out in ESS, and addressed with the new owner.
