1/29/2015	I met Nick Wilson to perform a well site survey on Wells #1, 2, and 3 at ~10:30 a.m. This follow-up visit was found to be needed due to reviewing the well information during the sanitary survey performed 1/13/2015. Well #1 was 123 feet to animal enclosures and 377.5 feet to the wastewater holding tank. Well #2 was 47 feet from a propane tank, 117 feet to animal enclosures, and 80 feet to a farm field owned by Sparboe Farms (Goodell). Well #3 was 57 feet to a farm field not owned by Sparboe Farms (Goodell) and 2 feet to a farm field owned by Sparboe Farms (Goodell). The bulk load out system was reviewed as well. They did not have a backflow preventer or an air gap on the system. Nick stated they do not use it anymore, and he would probably disconnect and cap it.	ASHLEY WALLER
