On May 15, 2015, I received a follow up response to the WS SS. I called Nick on May 21, 2015 to find out when he submitted his paperwork to obtain a Grade A certification. He stated he did not know, but it was a while ago. I called Jane Enfield, IDNR, she said his paperwork was processed on May 13, 2015.

I received an email with the attached spreadsheet of deficiencies on May 15, 2015 from Nick Wilson, Production Manager. For this one it stated, Oldson plumbing is coming Monday to install Morts has not came out to install yet so I have contacted Oldeson plumbing. 4/20/2015. The completion date was fill out with 4/20/2015.

Chlorine pumps and chlorine tests have been ordered along with flow meters to ensure that MORs can accurately be filled out correctly. MORs will be tracked and logged through a Sparboe SharePoint reminder workflow system. RPZs are not on the processing building and the production break room building incoming water lines. This should be installed on the processing building after the water goes through the domestic use areas, but before the water goes through the ion exchange softeners and on to the processing area. The production break room building should have one placed on the incoming water line prior to any domestic use. Subparagraph 567 of the Iowa Administrative Code 43.1(4) prohibits cross connections. Free and total chlorine residuals are not being taken at the source entry point, in the distribution system, nor when the bacterial sampling is being taken. They do not have a magnesium test kit or a working chlorine test kit. Subparagraph 567 of the Iowa Administrative Code 42.4(3)“a”(2) requires self-monitoring appropriate to the treatment process. Well #3 is 57 feet from a farm field not owned by Sparboe Farms (Goodell) and 2 feet away from a farm field owned by Sparboe Farms (Goodell). Well #2 is 80 feet from the field owned by Sparboe Farms Goodell. Please ask the farmer who owns the farm field 57 feet away from well #3 to not spray chemicals on his field within 43 feet of the property line by the well. Also, the farm field owned by Sparboe Farms (Goodell), but leased to a farmer. There shall be a stipulation in the leasing agree for the farmer to not spray chemicals on the field within a 100 foot radius of Wells #2 and 3. It is recommended wells meet the separation distances listed in Subparagraph 567 IAC 43.3(7), Table A. Wells that do not meet the distances may be prone to contamination and may require additional monitoring and/or treatment. The owner of a well constructed after 1979 is required to have legal control of the land for a 200-foot radius around the well through purchase, lease, easement, ordinance, or other similar means. A review of the Monthly Operation Reports (MORs) for the Sparboe Farms (Goodell) revealed the reports were not submitted for the past six months. During the sanitary survey, you had all of the MORs for the past year and provided me a copy. This is a violation of the Public Water Supply operation permit conditions and Subrule 567 IAC 42.4(3). The facility is required to submit MORs within 10 days after the end of each month. Continued failure to submit the MORs on time may result in a referral to the Department's legal staff for enforcement and disciplinary actions. Many facilities and operators find it helpful to fax or email the MORs to this office. The fax number is 641-424-9342. The MORs may be submitted as an email attachment to FO2.MOR@dnr.iowa.gov. An acknowledgement email is automatically mailed back to the sender, which provides confirmation that the email was received.

The liquid chlorine pump has been broken since at least August 2014. A new peristaltic chlorine pump is on order. Chlorine treatment is required per your permit at all times. Chemical feed pumps should be set at a feed rate no lower than 20% of its capacity. Operation in the middle third of the range is recommended. Recommended Standards for Water Works 5.0.3.g. Properly maintain the chemical feed equipment to ensure the proper feed rate can be maintained and to prevent this facility from allowing contaminants to enter the system. It is recommended tubing for peristaltic pumps be replaced according to manufacturer's instructions for proper operation of the pump.
