10/21/2013 reviewed suspense list and decided to close JANET GASTINEAU
6/27/2013 From: Gastineau, Janet [DNR] Sent: Thursday, June 27, 2013 10:22 AM To: Mike McCurnin; [email protected] Subject: sanitary survey These are all the outstanding questions I can come up with for now. Please provide the information in the next week or so. 1. How often is the tubing for the peristaltic feed pumps (sodium hypochlorite or others) changed? 2. What type of feed pumps are used for the fluoride? 3. What secondary control system is in place for the fluoride feed devise to reduce the possibility of overfeed? 4. What type of phosphate is used (ortho, ortho-poly blend, etc.)? 5. Please provide a written statement describing the limitations of the weekend sampling technician’s duties. 6. Please describe the radial collector well preventive maintenance program. 7. Last rate increase? 8. Completion date for switch to sodium hypochlorite? 9. Per the filter backwash rule, please provide the average and maximum backwash flow rate through the filters and the average and maximum duration of the filter backwash process in minutes; the typical recycle flow in gpm; the minimum plant rate during which the filter backwash was recycled; and confirmation of the frequency with which recycle flows are returned—all for the past year (June 2012—May 2013). If you think of anything else, please include it with your response. Bacterial sample collected was safe. Thanks you. JANET GASTINEAU
