10/29/2012	I visited this facility on 20SEP12 and found none of the deficiencies noted in the last inspection report had been addressed. Manager Mike Grell was away, but I called him on 24SEP12 and he indicated that he had just been too busy. I made it clear to him that these were priority tasks and he needed to get them done right away. See attached letter.	DAVID MILLER
