1/25/2016	Scott Wilson, Rachel Glaza and Perry Dargitz discussed the minimum total chlorine residual level. 7/27/2015	Response from Mr. Perry Dargitz states, "The outside contracted certified operator and myself have been working on increasing chlorine residuals through application of higher chlorine concentrations, installation of larger diameter pump tubing, and increasing the pump feed rates. Increased chlorine level measurements have been achieved, but still not to the minimum chlorine residuals as required under 567 IAC 42.4(3)"b"(1)"3". If the reliability of the chlorine feed systems to meet the minimum chlorine residual levels cannot be obtained, more robust systems will be evaluated to replace one or both feed systems in the powerhouse and maintenance areas. In reference to measuring total chlorine, instead of free chlorine, the facility's laboratory technician will change over the testing method to total chlorine starting August 1st. Additionally, the technicians will be switching the test methods for those sample results measuring over 2.20 mg/L of chlorine residual and retesting for the higher range utilizing the alternative method." 7/27/2015	Response from Mr. Perry Dargitz states, "I will be scheduling a time in August to take the qualifying examination for the Public water System Grade A Certification. I will be my intent to pass this examination on the first attempt. In the interim period beginning this week (July 15, 2015), a readily available contracted certified operator will be supporting the facility to assist in correcting the deficiencies identified in the Letter of Non-Compliance. This should serve as out operator certification compliance plan under Chapter 455B section 223 of the Code of Iowa."	ASHLEY WALLER
7/27/2015	Response from Mr. Perry Dargitz states, "The cross connection issue identified on the water line to the air conditioner in the water treatment room located in the maintenance building will have the appropriate preventive devices installed by August 15, 2015. In addition, the entire water distribution system will be walked down and other cross connection issues identified and corrected by the end of the year to meet requirements of Subparagraph 567 of the Iowa Administrative Code 43.1(4)." 			In the water treatment room located in the maintenance building, there is a water line connected to the air conditioner, which does not have a backflow preventer and an antisiphon device. A double check valve or a reduced pressure zone (RPZ) must be installed. Subparagraph 567 of the Iowa Administrative Code 43.1(4) prohibits cross connections. On the day of this inspection a cross connection was observed and discussed with Mr. Dargitz and Mr. Lind.
			The total coliform rule (TCR) sampling plan consisted of one sampling site and did not have any repeat locations. Please find three more sample sites, locate repeat locations, and update your sampling plan with this information. Major elements of the TCR plan shall include, but are not limited to, a map of the distribution system, notation or a list of routine sample location(s) for each sample period, resample locations for each routine sample, and a log of samples taken. 567 IAC 41.2(1)”c” & (2)2
			Mr. Dargitz is currently acting as the operator-in-charge, but is not properly certified. Also,He was informed he had to obtain a Grade A certification on January 22, 2014 by Mr. Carl Berg, IDNR. Mr. Dargitz has taken the Grade A class, but has failed to take the Grade A certification test. Chapter 455B section 223 of the Code of Iowa requires a certified operator for water treatment plants and water distribution systems. Subrule 567 IAC 81.2(3) requires these operators to be certified at the same grade or greater than the water treatment plant and distribution system. Compliance with the operator certification rules can be achieved by: • Employing an operator having the required certification. • Having an existing employee obtain the required certification. • Obtain the services of a properly certified operator by affidavit or by contract with a company suppling operator services. You must respond in writing by July 8, 2015 as to the course of action you plan to take to bring your facility into compliance with the operator certification rules. Your written response will serve as your operator certification compliance plan. Failure to obtain the services of a properly certified operator or submit an approvable operator certification compliance plan may result in the Department taking enforcement action against your facility which can include an administrative order and monetary penalty.
			Well #1 and 2 have an airline installed for static and pumping levels, but it is unknown if they are still in working order. A static and pumping water level test must be conducted to determine if they are still in working order. Static and pumping water levels must be measured and recorded once per month for all active groundwater sources unless otherwise approved by the Department. If an airline is no longer functioning, it must be replaced the next time the pump is pulled for repairs. 567 IAC 42.4(3)"a"(2)6
			There are two underground fuel tanks located east of well # 1. During the sanitary survey conducted on 10/05/2012, Mr. William Bertie, retired operator-in-charge, reported the tanks were installed in 1993 and are equipped with leak detection equipment. Mr. Bertie measured the distance from the nearest diesel fuel tank with an engineer’s wheel on 02/01/10 and reported the distance is 163 feet. The separation distance requirement is 200 feet from deep wells. Wells that do not meet the distances may be prone to contamination and may require additional monitoring and/or treatment. Therefore, Well #1 does not meet the separation distances and additional monitoring will likely be required in accordance with Chapter 567 IAC 41.5.
			A minimum free chlorine residual of 0.3 mg/L or a minimum total chlorine residual of 1.5 mg/L must be continuously maintained throughout the distribution system except for those points which terminate in dead ends or are very low use areas. 567 IAC 42.4(3)“b”(1)“3”
