1/25/2016 Perry came into Field Office #2 on 1/15/2016 and provided a photo of the installed RPZ. RACHEL GLAZA
1/25/2016 Scott Wilson, Rachel Glaza and Perry Dargitz discussed the minimum total chlorine residual level. Scott suggested Perry adds more chlorine and double checks his testing method at injection sites. Scott also suggested dilution sampling to obtain accurate readings of total chlorine. RACHEL GLAZA

7/27/2015 Response from Mr. Perry Dargitz states, "The outside contracted certified operator and myself have been working on increasing chlorine residuals through application of higher chlorine concentrations, installation of larger diameter pump tubing, and increasing the pump feed rates. Increased chlorine level measurements have been achieved, but still not to the minimum chlorine residuals as required under 567 IAC 42.4(3)"b"(1)"3". If the reliability of the chlorine feed systems to meet the minimum chlorine residual levels cannot be obtained, more robust systems will be evaluated to replace one or both feed systems in the powerhouse and maintenance areas. In reference to measuring total chlorine, instead of free chlorine, the facility's laboratory technician will change over the testing method to total chlorine starting August 1st. Additionally, the technicians will be switching the test methods for those sample results measuring over 2.20 mg/L of chlorine residual and retesting for the higher range utilizing the alternative method." 7/27/2015 Response from Mr. Perry Dargitz states, "The total coliform rule (TCR) sampling plan will be updated by the end of the month with three more sample sites, repeat locations, and other appropriate information required by Chapter 567 IAC 41.2(1)"c"&(2)2." ASHLEY WALLER
7/27/2015 Response from Mr. Perry Dargitz states, "I will be scheduling a time in August to take the qualifying examination for the Public water System Grade A Certification. I will be my intent to pass this examination on the first attempt. In the interim period beginning this week (July 15, 2015), a readily available contracted certified operator will be supporting the facility to assist in correcting the deficiencies identified in the Letter of Non-Compliance. This should serve as out operator certification compliance plan under Chapter 455B section 223 of the Code of Iowa." ASHLEY WALLER
7/27/2015 Response from Mr. Perry Dargitz states, "The cross connection issue identified on the water line to the air conditioner in the water treatment room located in the maintenance building will have the appropriate preventive devices installed by August 15, 2015. In addition, the entire water distribution system will be walked down and other cross connection issues identified and corrected by the end of the year to meet requirements of Subparagraph 567 of the Iowa Administrative Code 43.1(4)."
