6/28/2017	A storm water investigation was conducted on June 22, 2017. It was conducted by Rick Martens and Brian Jergenson of Field Office One. A notice of violation was issued for 567 IAC 64.3 - Permit to Operate. Walz Energy LLC was conducting construction activities without a storm water permit.	RICK MARTENS
