5/8/2012 Compliance inspector, Steve Ellsbury, did not receive response form for the above site. This site requires follow up to see if compliance items have been resolved. 6/6/2012 Visit to Amoco Pronto. Spoke to acting manager Darla Rickers who knew nothing about the deficiencies from the November inspection. A call was placed to Jay Sexton who stated that the shear valve that was tied open was scheduled to be repaired next week but that the other things had been tken care of. He was informed that records were not present on-site to verify this. Mr. Sexton was given until June30, 2012, to submit the completed response forms to the inspector and a copy to the field office for both this site and the Mapleton Amoco which he had been contacted about on 5/14/2012.
