8/26/2011	After a site visit by B. Gibbons, FO5, it was determined material could be recovered from the bank and sand bar. Mr. Gibbons requested they recover as much of the lime as possible.	ADAM BROUGHTON
8/26/2011	Bill Gibbons and Janet Gastineau went to the site on 8/26/2011. Des Moines Water Works used a skid loader and drag line. The Skid Loader scraped up the material and piled it up on the river bank. The drag line moved the material from the river bank to the upper bank. The material was loaded into dump trucks using a track hoe. It took three dump truck loads to haul the material away. It was incorporated into on-site construction materials. Per R.W. Smith the Safety Director the material was analyzed by the lab and found to be strictly calcium carbonate.	BILL GIBBONS

			On August 30, 2011 we sent a Notice of Violation and required Des Moines Water Works to do the following: By September 30, 2011 submit a written response to this office addressing the specific measures that will be taken by the Des Moines Water Works to prevent the future release of lime waste from the dry well at the flocculation unit(s) including a time line for implementation.
