12/2/2019	During the complaint investigation we documented manure from the recent land application flowing into an unnamed tributary of the East River along the east side of the field and directly into the East River from the southern portion of the field. Additionally, manure was pooled behind several tile inlet terraces and because the manure was not properly incorporated, separation distances to a water source and a nearby residence were not met. Jason Larabee, manure applicator, was directed to immediately incorporate the manure. The manure originated from the Utah North facility (#66415). The application equipment was still at the site. One piece of equipment was leaking manure and was not properly marked showing who the applicator was. Additionally, manure was pooled up all along the north side of the confinement. Mr. Larabee was directed to immediately excavate the manure.	ALISON MANZ
