9/8/2015	On 8/20/2015, I called Brenda Linquist, Sparboe Farms Corporate Office Manager, to inform her of the MOR continually being submitted late and an NOV was going to sent. I informed her we needed to ensure the corporate office was aware of this issue. She stated to send her a copy of the NOV letter. She stated Nick Wilson is now a Project Manager for Sparboe Farms, Inc., but is based out of the Goodell site. She stated he is gone approximately 2 weeks out of a month. I called Nick Wilson to confirm this information. He stated he did receive a promotion, but would still be available to take care of the water system for the Goodell site.	ASHLEY WALLER
8/17/2015	During a 6-month review of MORs for timeliness, I discovered Sparboe Farms Goodell did not turn in May and June MORs and were late turning in February's MOR. I called Nick Wilson, Operator, and stated they will receive another NOV for MORs due to this.	ASHLEY WALLER
