It appears something happened with the formatting of the Excel form. I have attached a screenshot of your February DMR with the red columns circled. I have also attached a new Excel DMR form (DMR_5500112.xlsx) for you to use going forward. Only your data should be put into this new form. Please do not try to edit any formatting or formulas. When you submit your DMR there should not be any red boxes in Row 15. There either needs to be data in the column, or a reason picked from the dropdown menu in row 15 for why there is no data.
