2/16/2011	The operation permit was allowed to expire in 2005 and was not tracked by the department so a permit renewal reminder was not generated. I visited with Stoney Hinshaw, Complex Manager, about applying for an opeartion permit and to check the status of the facility. The facility continued to operate under the conditions of the expired permit, not knowing that it was expired.	TOM ATKINSON
