4/22/2016	Aeration header in aeration basin of WWTP broke causing 60,000 gallons of activated sludge/mixed liquor to be discharged to the creek via a storm drain. Solids evident and fish kill from discharge to confluence with the Des Moines River. Worked with Rathbun Fisheries staff to conduct a fish count and assessment.	TOM ATKINSON
