For this one it stated, Oldson plumbing is coming Monday to install Morts has not came out to install yet so I have contacted Oldeson plumbing. 4/20/2015. 			RPZs are not on the processing building and the production break room building incoming water lines. This should be installed on the processing building after the water goes through the domestic use areas, but before the water goes through the ion exchange softeners and on to the processing area. The production break room building should have one placed on the incoming water line prior to any domestic use. Subparagraph 567 of the Iowa Administrative Code 43.1(4) prohibits cross connections. On the day of this inspection a cross connection was observed and discussed with you. Please respond within 30 days of the receipt of this report as to the changes that will be made and a time schedule for implementation to eliminate the cross connection.
			Free and total chlorine residuals are not being taken at the source entry point, in the distribution system, nor when the bacterial sampling is being taken. They do not have a magnesium test kit or a working chlorine test kit. Subparagraph 567 of the Iowa Administrative Code 42.4(3)“a”(2) requires self-monitoring appropriate to the treatment process. The chlorine residual shall be measured and recorded daily at the S/EP and at representative points in the distribution system and at the same time coliform bacteria samples are collected for compliance purposes. 567 IAC 42.4(3)"a”(2)5 and 43.6(1)"c"(2)1. A test kit capable of measuring free and combined chlorine residuals in increments no greater than 0.1 mg/L in the range below 0.5 mg/L, and in increments no greater than 0.2 mg/L in the greater than 0.1 mg/L in the range below 0.5 mg/L, and in increments no greater than 0.2 mg/L in the range from 0.5 mg/L to 1.0 mg/L, and in increments no greater than 0.3 mg/L in the range from 1.0 mg/L to 2.0 mg/L must be provided at all chlorination facilities. 567 IAC 42.4(3)"b"(1)4. Please respond within 30 days with how you intend to comply with this requirement.
			Chapter 455B section 223 of the Code of Iowa requires a certified operator for water treatment plants. Subrule 567 IAC 81.2(3) requires these operators to be certified at the same grade or greater than the water treatment plant and water distribution system. On the day of this inspection, this treatment plant did not have a properly certified operator. Please complete and submit Form 52 within 30 days, stating how you will obtain a certified operator.
			Well # 1, 2, and 3 do not have a flow meter prior to treatment. Please place a flow meter after all three wells have been blended together prior to being treated with liquid chlorine and going through the iron filtration system. Recommended Standards for Water Works 3.2.7.3.a.4 requires well discharge piping be equipped with a means of measuring flow be provided. Please respond within 45 days of receipt of this report as to when and how this deficiency will be corrected.
			Well # 1, 2, and 3 are not being tested for static and pumping water levels. They are all equipped with the drawdown lines, but they do not have a pressure gauge attached. A pressure gauge shall be procured. Static and pumping water levels must be measured and recorded once per month for all active groundwater sources unless otherwise approved by the Department. If an airline is no longer functioning, it must be replaced the next time the pump is pulled for repairs. 567 IAC 42.4(3)"a"(2)6
			Well #3 is 57 feet from a farm field not owned by Sparboe Farms (Goodell) and 2 feet away from a farm field owned by Sparboe Farms (Goodell). Well #2 is 80 feet from the field owned by Sparboe Farms Goodell. Please ask the farmer who owns the farm field 57 feet away from well #3 to not spray chemicals on his field within 43 feet of the property line by the well. Also, the farm field owned by Sparboe Farms (Goodell), but leased to a farmer. There shall be a stipulation in the leasing agree for the farmer to not spray chemicals on the field within a 100 foot radius of Wells #2 and 3. It is recommended wells meet the separation distances listed in Subparagraph 567 IAC 43.3(7), Table A. Wells that do not meet the distances may be prone to contamination and may require additional monitoring and/or treatment. The owner of a well constructed after 1979 is required to have legal control of the land for a 200-foot radius around the well through purchase, lease, easement, ordinance, or other similar means. Please respond within 45 days of the receipt of this report as to how legal control will be obtained. 567 IAC 43.3(7)"b"(1)3
			There is a bulk loading site on 3 North Barn without an air gap or backflow preventer. This system must ensure all bulk loading sites provide positive separation from the potable water supply system through the use of an air gap or backflow preventer. If you are not going to use it anymore, it needs to be taken out of the system and capped. Within 45 days this system must submit a written confirmation to this office that confirms that appropriate measures have been taken to provide a positive separation between bulk loading sites and the potable water system. 567 IAC 43.1(4)“b”.
			A minimum free chlorine residual of 0.3 mg/L or a minimum total chlorine residual of 1.5 mg/L must be continuously maintained throughout the distribution system except for those points which terminate in dead ends or are very low use areas. 567 IAC 42.4(3)“b”(1)“3”
			The system is reminded all bacteriological analyses records; all monthly records of operation completed as described in 567 IAC 42.4(3), Appendix B and 43.5(4) & (5); and all copies of reports, summaries or communication related to reports concerning variances or exemptions must be retained for a period of not less than five years. [567 IAC 42.5(1)].
			The system is reminded all records of chemical analyses; all Sanitary Surveys; and copies of all reports, summaries or communication related to Sanitary Surveys shall be retained for a period not less than ten years. [567 IAC 42.5(1)]
			The system is reminded all copies of reports, summaries or communication related to copies of public notices issued, and all records of actions taken to correct violations must be retained for a period not less than three years. [567 IAC 42.5(1)]
			There were some loose bolts on well #2 and 3, and they should be tightened. Recommended Standards for Water Works 3.2.7 requires wells be sealed properly to prevent the entrance of contamination.
			It is unknown where the iron filtration backwash is discharged. Please locate where the iron filtration backwash is discharged. If you are going to dye test the iron filtration discharge system, please contact Connie Dow at 515-281-3350 or connie.dow@dnr.iowa.gov, for the dye test procedures and requirements.
			A review of the Monthly Operation Reports (MORs) for the Sparboe Farms (Goodell) revealed the reports were not submitted for the past six months. During the sanitary survey, you had all of the MORs for the past year and provided me a copy. This is a violation of the Public Water Supply operation permit conditions and Subrule 567 IAC 42.4(3). The facility is required to submit MORs within 10 days after the end of each month. Continued failure to submit the MORs on time may result in a referral to the Department's legal staff for enforcement and disciplinary actions. Many facilities and operators find it helpful to fax or email the MORs to this office. The fax number is 641-424-9342. The MORs may be submitted as an email attachment to FO2.MOR@dnr.iowa.gov. An acknowledgement email is automatically mailed back to the sender, which provides confirmation that the email was received.
			The floor of the chlorine/pressure tank room is covered in approximately one inches of water with a bloated mouse floating in it. This room should be drained of water and cleaned. The integrity of the leaking storage facility should be inspected to determine where the water is coming from. It is undetermined at this time if the leaking is coming from the pressure tanks or if it is coming from somewhere else
			The liquid chlorine pump has been broken since at least August 2014. A new peristaltic chlorine pump is on order. Chlorine treatment is required per your permit at all times. Chemical feed pumps should be set at a feed rate no lower than 20% of its capacity. Operation in the middle third of the range is recommended. Recommended Standards for Water Works 5.0.3.g. Properly maintain the chemical feed equipment to ensure the proper feed rate can be maintained and to prevent this facility from allowing contaminants to enter the system. It is recommended tubing for peristaltic pumps be replaced according to manufacturer's instructions for proper operation of the pump.
