			Summary of Significant Deficiencies 1C: Recommended Standards for Water Works 3.2.4.10.f requires that wells be sealed properly to prevent the entrance of water or other contamination. The bolts for Well #2 were loose and must be tightened. The well cap for Well #4 was cracked and must be replaced. 2A: On the day of the sanitary survey inspection cross-connections were observed in the treatment plant. The softener discharge line did not contain an air gap. Additionally, a garden hose was attached to a water line in the plant and the hose was in contact with the ground. Subrule 567 IAC 43.1(4) requires cross-connection control to prevent backsiphonage or backflow of contaminants into the public water supply.
			Summary of Minor Deficiencies 1D: Static and pumping water levels must be measured and recorded once per month for all active groundwater sources (Well #2 and Well #4) unless otherwise approved by the Department. If the existing airlines are no longer functioning, they must be replaced the next time the pumps are pulled for repairs. [567 IAC 42.4(3)"a"(2)6] 1F: The standby or restricted use source (Well # 3) must be adequately maintained to prevent degradation of the groundwater. The pump must be routinely exercised to ensure the source remains viable. A well not being maintained properly or not exercised regularly shall be considered abandoned. Abandoned wells must be plugged according to 567 IAC Chapter 39. 6B: Facility staff at the time of inspection were unclear of the location of the Source/entry point. The Source/entry point must be clearly identified. Source/entry point samples must be taken at a designated location that is representative of all water after treatment and prior to entering the distribution system.
