7/19/2016	Recent complaints against the facility, and the missing documents that came to light during my last visit (#108997), were discussed. Dan explained that the missing documentation was due to the resignation of his equipment operator during the early spring of this year and that it is very unlikely that would ever happen again. I advised close supervision to ensure compliance in the future.	ADAM SHAFFER
