4/17/2014	On February 12, 2014 in response to a request by Susan Johnson of the DNR CO SW Section, I returned to the Gubbels One Hour Heating facility in Waterloo to verify that all appliance demanufacturing activities had stopped and that all regulated components had been shipped out. This would allow Susan to close out this facility's ADP permit as per their recent request. I met with Justin Schrader and Russell Schrader on this date. We toured the site. I was informed that their last shipment of flourescent tubes, capacitors, and mercury containing components were shipped to A-Tex Recycling Inc. of Desmoines back on 5/20/13. I determined that this facility is no longer demanufacturing appliances and that their ADP permit can be rescinded. This information was forwarded on to Susan Johnson via email and this permit was subsequently rescinded. No further DNR actions required.	DOUG HAWKER
