1/26/2012	Clay Greenawalt completed the CRt registration for and sent it to FO2. We sent it to Des Moines.	DAVID MILLER
1/9/2012	I had inspected this appliance demanufacturing facility on 12DEC11 and there were no cathode ray tubes (CRTs) on site at that time. I called Clay Greenawalt on 9JAN12 and he stated that one of the dealers from whom he obtains his appliances for demanufacturing is making him take the televisions as part of the deal, and that he had collected four televisions. I advised Mr. Greenawalt of the CRT collection point registration requirements under 567 IAC122 and will send him the CRT registration form. Mr. Greenawalt said he was not previously aware of this, but would comply now that he knew. We discussed that he needed to take the CRTs to a proper disposal facility and Mr. Greenawalt acknowledged this.	DAVID MILLER
