4/16/2014	Investigation conducted in conjunction with IDALS Compliance Investigator Doug Anderson. After extensive interview of site owner, both Investigator Anderson and myself concluded this was a one-time, honest mistake by a new employee and we have no reason to suspect it will happen again. We found no evidence of an actual violation of IAC regulations.	TRENT LAMBERT
