This is the first request I have received for the application materials. I suggest contacting David Schelling by electronic mail; I have included him on this email. However, the forms you need are located at http://www.iowadnr.gov/Environmental-Protection/Water-Quality/NPDES-Wastewater-Permitting/NPDES-Forms/NPDES-Application-Forms#Industrial. David can let you know which ones are needed for this discharge or you can refer to the renewal application submitted for the LP Moon discharge and select the same forms. The Des Moines Water Works is entering the final stages of construction of the Army Post Road Aquifer Storage and Recovery Well. In talking with our operations staff, it sounds like we will need to apply for an operating permit for the well prior to recovering previously injected drinking water and sending it to our distribution system. Could you direct me to where I could get a copy of the permit application and the application process/requirements? Thank you.
