4/4/2017 2/6/2017 1654: Received a call on the HSI phone from Brian Stumpf of the Des Moines Water Works regarding a water main that was hit by farm equipment resulting in a water main break with depressurization (uncontrolled). The location was on NE 52nd Avenue with an address of 5301 NE 110th Street, Beaver Township (West of Mitchellville). The water supply was notified at 1330. They isolated and replaced the pipe and will issue a boil order on their code red system. They have 4 rural connections. Based on the information they supplied they would need to do one sample immediately following the repair and then another two sixteen hours later. Mr. Stumpf mentioned that when the samples come back as bacterially safe they will lift the BA via their code red public notice system. I notified the appropriate field office (FO5) as well as the eastern region field office via e-mail at 8:07 on 2/7/17. JON RYK
