1/26/2016 Menefee left a phone message stating that Tendall had mailed the application the day before our conversation, so the issue should be resolved very soon. SCOTT WILSON
1/22/2016 I called Menefee (the corporate environmental manager) because they do not have a properly certified operator in Britt at this time. Mark Tendall is eligible for the Grade A certification, but he has not submitted the required application. Menefee will immediately check on the situation and submit the paperwork. We also discussed two other options. They could hire an affidavit operator, or Menefee could be the operator if he is physically at the site once or twice per week. Menefee said that he has a Grade 1 certification. We reviewed the definition of "operator in charge." Since Menefee is not able to be onsite weekly, he will have one of the onsite staff persons (Tendall) be the OIC . SCOTT WILSON
