1/25/2016	On 1/15/16 Scott Wilson also discussed with Perry the instability of chlorine residuals. Perry indicated he would be aware of the levels and try different tactics to stabilize the chlorine residuals.	RACHEL GLAZA
1/19/2016	I sent Perry an email on 1/14/16 inquiring about the deficiencies listed on the Water Supply Sanitary Survey conducted on 6/17/15. There were 3 significant deficiencies that needed to be addressed. Perry came into the office on 1/15/16 at about 1:30pm to address deficiencies. Scott Wilson and I assisted him. Perry submitted information to satisfy TRC and GWR. He also provided evidence of eliminating cross connections in the system. Perry obtained operator certification (water treatment grade A) on 8/25/15. Perry also provided a certificate for Gregory Lee, another operator at the facility. Gregory Lee has a water treatment and distribution grade 4 certificate.	RACHEL GLAZA
