10/13/2015 From: Miller, David [DNR] Sent: Tuesday, October 13, 2015 7:09 AM To: Hutchins, Brian [DNR] Cc: Seda, Ann [DNR]; Carper, Glenn [DNR] Subject: Lehigh Mason City I gave Mr. Dargitz one week to produce some missing information. This is the 3rd week and I have, in the interim, sent out the report with NOVs for most of these. DAVID MILLER Environmental Specialist Senior ++++++++++++++++++++++++++++++++++++++++++++++++++++++++++ From: Dargitz, Perry D (Mason City) USA [mailto:Perry.Dargitz@LehighHanson.com] Sent: Monday, October 12, 2015 4:50 PM To: Miller, David [DNR] Cc: O'Neill, Tom (Mason City) USA; Sterner, Richard (Allentown) USA Subject: Reply on questions Dave, Here is the follow-up comments to your questions from the recent Title V inspection: 1) The reason for the highlighted blue numbers on the total daily and total year to date monthly throughput spreadsheets is these numbers are “hard punched” into the spreadsheet from information off the quarry daily reports. All the other numbers on the sheet are automatically transferred from other spreadsheets and data bases. 2) Transfers from sources 75A and 76A are controlled through the documentation on the quarry daily report. These are then transferred into the monthly throughput spreadsheets. 3) There are spreadsheets designed for the daily number of round trips on the limestone haul road, clay haul road, and paved haul road from the product loadout silos. The yearly totals for these trips will be tabulated for the last four years and the present year. We will design the spreadsheets to automatically calculate the year to date total counts. Also, spreadsheets have been designed for maintaining the appropriate informational records for controlling fugitive emissions from these roads and will be fully utilized by the end of November. Attached is the spreadsheets for the daily truck trip counts with the limestone haul road trip counts highlighted in green, clay haul road trip counts highlighted in orange, and the paved haul road at the loadout silos found on the second spreadsheet. 4) The completed rolling 12-month averages will be completed by the end of November for the following: a. Conveying System Transfer Points/Raw and Finish Mills/Storage Bins/Bulk Loading and Unloading Units (EP11, EP18-23, EP41-46, EP39) b. Non-Fugitive Sources (EP8-10, EP12-13, EP15) c. Crushers and Raw Material Transfer (EP2-3, EP5-6) 5) The regulation 40 CFR 63.1350(f) is being followed according to opacity for these emissions. I will organize these to emission categories in the Title V permit and provide these sheets with explanations this week. 6) EP13 is incorrectly identified in the permit. Should be Raw Material Transfer-Crusher/Stone Belt 2 to Crusher Belt 3, not 2. I need to find out if this emission point is still operational or inactive. 7) The coal mill EP62 has not operated this year, but only a couple days during engineering stack testing in preparation for the new NESHAP PC MACT Rule changes.
