8/17/2015	On 08/17/2015 at ~0922 Mr. Sample contacted the Field Office with question regarding maintenance. Mr. Sample explained that there was a small leak in a pipe near the injection point for the hypo-chlorination. Mr. Sample wanted to know if there were any forms that had to be filled out or other paperwork, and if a boil advisory was required. Mr. Sample a was asked if there would be a pressure loss. According to Mr. Sample they would isolate ~20 feet of pipe, and they would have to de-pressurize the line to repair it. Mr. Sample was informd that they could limit the boil advisory to the area affected and collect samples. Mr. Sample was also asked why the facility has not developed a plan of action for such work. It was mentioned that that the facility has contacted the Field Office numerous times over several years regarding such work. Mr. Sample indicated that he was new to the position and maybe something he may need to work on.	MARK HEIDERSCHEIT
