5/9/2013 On 05/09/2013 at ~1100 Mr. Leckband contacted the Field Office and requested to speak with Mr. Jon Ryk regarding MOR's and reporting flows. Due to Mr. Ryk being in the field, Mr. Leckband was directed to me. Mr. Leckband attempted to explain the issue. Per the conversation, the facility had stopped reporting flows on the MOR's for all the wells last month. Mr. Leckband indicated that they were adding the wells back on the MOR's and wanted to know if they should amend the last MOR and re-submit it. Mr. Leckband indicated that the other wells were used for production, but that there was a check valve between the one well and the potable system. Mr. Leckband also mentioned an issue with a wash station. Per Mr. Leckband, there was an air gap that was not noted on the schmeatic or block diagram that previously provided. Mr. Leckband indicated that they have pictures. It was suggested that they go ahead and modify the MOR to include the wells that were not being reported, submit it, and follow up with Mr. Ryk since I was not clear on what was happening.
