12/1/2011	Determined later that excess emissions did indeed occur. A separate incoming report for excess emissions was filled out for this incident.	GLENN CARPER
11/9/2011	Keith called to report that the RTO was taken down because they could not properly regulate the heat. They discovered that the refractory inside the unit was damaged and will have to be replaced. He said he would let me know when it was back on line. He stated that this problem did not cause any excess emissions.	GLENN CARPER
