6/1/2009	Dawson received a letter stating that he had not completed his public notice. He said he had sent it in and he had the certified mail receipt. I informed him that I would have to check with our Des Moines office and call him back. I checked with Marie Leat and she said that they had received his PN for the recent positive bacti but that the most recent letter he received was due to a monitoring violation approximately a year ago. According to Marie a recent EPA audit noted that the department had not received responses for several PN. In response, letters were sent to supplies that had not returned their PN info. I called Dawson back and explained the situation. He said that he never received the PN for the monitoring violation. He said that there was a mix up with the lab and that issue had been resolved. I explained that I would have to pull the file and check with Marie. I checked with Marie and she said the issue with the lab was never resolved.	DAVID HOPPER
