12/27/2010	On 12/27/2010 at ~1515 Ms. McIvor called in regards to a modification to the system. Ms. McIvor indicated that they were going to install a new production well and a header. The system is currently down and they would like to install the header while the system is down. She asked if a construction permit was required. She was asked if the production well would be connected to the public water supply, and the response was yes. She was informed if it was connected with the public water supply they would probably need to have a permit. She was instructed to contact Water Supply Engineering to discuss it further. The phone numbers for Daryl Enfield, Fengua Yang, & Michael Anderson were provided.	MARK HEIDERSCHEIT
