4/9/2009 On Thursday, April 9, 2009 at 1:30 PM Tim Elder with Elder Corporation (515-266-3111, cell 515-249-1453) stopped at Field Office 5 to drop off a Notification of Tank Closure or Change-in-Service form for removal of tanks and piping at Don's East Grand Conoco at 201 East Grand Avenue in Des Moines (UST Registration No. 198608863). Tim said he was not sure where to turn in the form. I told him usually it should go to the Wallace Building, attention Tom Collins or Paul Nelson. I told him I could fax the form to Tom Collins since he dropped it off at the Field Office (I checked with Paul Nelson to make sure a fax copy was OK). I had Tim sign and date the form and I faxed it to Tom Collins with the Iowa DNR UST Section. I asked Tim when they planned on removing the tanks. He said they were going to start tomorrow (Friday, April 10, 2009). I told Tim that usually we like to receive the written notification a month in advance and then they needed to call the Field Office 24 hours before they were going to remove the tanks. I said since he was here at the Field Office the day before the removal that I would take that as his 24 hour notice. Tim told me Allender Butzke (Donald Edds - CGWP 1298) was going to collect the tank closure samples. He said he had also contracted to have the tank properly cleaned out and scrapped and would obtain a certificate of proper disposal. I sent an e-mail to Tom Collins and Paul Nelson with the DNR UST Section and also to Bill Gross, Janet Gastineau, and Jim Stricker with Iowa DNR Field Office 5. The E-mail went as follows: Tom, Tim Elder with Elder Corporation just stopped at Field Office 5 (1:30 PM – Thursday, April 9, 2009) to drop off the Notification of Tank Closure for UST Reg. No. 198608863, Don’s East Grand Conoco at 201 East Grand Avenue in Des Moines. I faxed a copy of the notification to you. Don said they would start tank and piping removal tomorrow (Friday, April 10, 2009). Since he stopped at the field office I took that as their 24 hour notification to the local field office. Allender Butzke (Don Edds - #1298) will be doing the tank closure sample collection. Sincerely, Bill Gibbons BILL GIBBONS
