6/2/2021	I have not received the written response despite two phone calls and an email requesting the information. Mark verbally stated the deficiencies were corrected, but no written response has been submitted. The matter is considered closed as the deficiencies are primarily record keeping issues and the facility is aware of the improvements needed and has verbally stated they will be corrected.
