9/4/2020	Heather Brown called regarding their toxicity testing reporting requirements. In 2019, did not submit form 542-1381 with her results but did report them on her DMR. In 2020, she will submit the form for 2019 and 2020 along with reporting the results from 2020 on the DMR. She also questioned whether she needed to submit the results with her permit application. After discussing with Ryan Olive, she will need to submit Form 30, Part C: Toxicity Testing Data with her permit application.	JACOB DONAGHY
