6/22/2018 After a significant oil spill from this facility, Hylton Jackson, DNR Contaminated Sites Section, became involved and we combined this most recent spill with the existing work plan. Following a meeting between CHS and DNR staff, a new work plan was requested documenting the location of additional monitoring wells. The work plan is in the 'attached documents' and in the facility file. Response from Hylton: Mr. Tucker, I have reviewed the work plan for the monitoring well installation and monitoring/free product gauging at the NuStar facility (formerly CHS) in Council Bluffs. The plan is approved with the following stipulations; The locations and construction plans for the three new monitoring wells is approved. (Please note that, pending the sampling results, additional wells and/or remedial actions may be required. The monitoring, gauging and free product recovery plan is approved. An initial round of groundwater samples will be collected and analyzed from all wells (sample analysis will not be performed on any well containing LNAPL). Monthly groundwater gauging will occur on the 13 monitoring wells and 6 recovery wells specified in the plan. All wells located at the facility shall be maintained until further notice. A release of diesel fuel from tank # B 102 was reported in 2017. If this tank is removed or replaced, additional soil sampling will be performed and any appropriate remediation measures will be taken. Please inform the Department prior to any removal or replacement. Future groundwater monitoring requirements will be determined after review of the acquired data. ALISON MANZ
