6/15/2018	Jeff called in to ask how to send in his short form MMP update. This is due to the fact that he changed consultants. I directed him to the website where he could use the form number to search for and print off a copy of the form, fill it out, and send it with a check here to the field office. I looked up his facility in the AFO database and let him know that a short form is correct for this year and due by September 1st.	ADAM SHAFFER
