Member Login



Description:
Provides access into the secure area of the Private Well Tracking System system. User must have a valid login id and password.
 
Fields:
Username: (Required) The username of the individual logging into the member's area. To request a username or obtain assistance with forgotten passwords, contant Erik Day at the DNR.
Password: (Required) Secure password for the username entered above. A minimum of five characters must be entered.
Remember me next time: If this box is checked when you log in, then you will not need to log in again next time.
 
Buttons:
Log In: Attempts to log the person into the system. If successful, the user will be returned to the menu with a new menu list provided access to the secure portions of the system.

Forgot Your Password?: Takes the user to the Password Retrieval screen.

Password Retrieval



Description:
Resets a user's password to a random set of characters and e-mails it to them. After logging in with the new password the user can go to the Change Password page in the member's area.
 
Fields:
Username: (Required) The username of the individual who needs their password reset and sent to them. An e-mail address must have been provided for the user.
 
Buttons:
Submit: Attempts to send the new password to the user. A confirmation or an error message are displayed.

Change Your Password



Description:
Provides a method for the user to change their password while logged into the system.
 
Fields:
Password: (Required) The user's current password that was used to log into the system. A minimum of five characters must be entered.
New Password: (Required) The new password that the user would like to change to. A minimum of five characters must be entered.
Confirm New Password: (Required) The new password that the user would like to change to (re-type to confirm spelling). A minimum of five characters must be entered.
 
Buttons:
Change Password: Submits the new password to the system. If all of the information is valid, then the user will be taken to the home page. If not, an error message will be displayed.
Cancel: Cancels out of the password change process and returns the user to the home page.

Permit Search



Description:
Search for an existing permit. The user can get the permit information by entering the appropriate search criteria, which includes permit details, property owner information and well location details and clicking on the search button.
 
Fields:
Permit Number: The unique number generated by the system for identifying each permit.
Permit Date: The date that the permit was issued.
Certified Well Driller: The name and current certification number of the well driller who is likely to drill the well(s) listed in the permit.
Expired Well Driller: The name and expired certification number of the well driller who was likely to drill the well(s) listed in the permit. This applies if the well driller had a certification number that is now expired at the time when the permit was issued.
County: The name of the county where the well(s) listed in the permit are located.
First Name: The property owner’s first name.
Middle Name: The property owner’s middle name.
Last Name: The property owner’s last name.
Company: The name of the company.
Quarter 5: The quarter of the quarter of the quarter of the quarter of the quarter of the section, township, range of the well.
Quarter 4: The quarter of the quarter of the quarter of the quarter of the section, township, range of the well.
Quarter 3: The quarter of the quarter of the quarter of the section, township, range of the well.
Quarter 2: The quarter of the quarter of the section, township, range of the well.
Quarter 1: The quarter of the section, township, range of the well.
Section: The section of the well location.
Township: The township of the well location.
Range: The range of the well location.
Direction: The direction of the well location.
Parcel Number: The value used at the county level for identifying a well location.
E911 House Number: The house number of the E911 address of the well.
E911 Street Address: The street address of the E911 address of the well.
 
Buttons:
Submit: Takes the user to the Permit Overview screen, which displays a list of header information for the permits that match the search criteria.
Reset: Resets all of the fields to their default value.

Permit Overview



Description:
Displays a list of permits matching the search criteria entered by the user in the Permit Search screen. The user can get the details of a particular permit by clicking on the permit number.
 
Fields:
Permit Number: The unique number generated by the system for identifying each permit. Clicking on the permit number takes the user to the permit's details.
Permit Date: The date that the permit was issued.
Property Owner: The name of the person or company that owns the property.
Status: The status of a permit, which can be Active, Completed, Log Completed, Water Test Completed, Expired or Withdrawn.
County: The name of the county where the well(s) listed in the permit are located.
Certified Well Driller: The name and current certification number of the well driller who is likely to drill the well(s) listed in the permit.
Expired Well Driller: The name and expired certification number of the well driller who was likely to drill the well(s) listed in the permit. This applies if the well driller had a certification number that is now expired at the time when the permit was issued.
Well Location: The PLS coordinates of the well location (QQQQQSTR).
 
Buttons:
Return: Takes the user back to the Permit Search screen.

Permit Details



Description:
This purpose of this screen is to maintain permit information. The user can create, view, update or delete permit information depending on their privileges.
 
Fields:
County Name: (Required) The name of the county where the well(s) listed in the permit are located.
Well Purpose: (Required) The purpose of the well(s) in the permit.
Permit Number: The unique number generated by the system for identifying each permit.
County Agent: (Required) The name of the county agent issuing the permit.
Permit Date: (Required) The date that the permit was issued.
Contract Company: The name of the contractor company the well driller is associated with. Along with the contractor company name, the city in which it resides is also displayed.
Well Driller: (Conditional) The name and current certification number of the well driller who is likely to drill the well(s) listed in the permit. This is required if the expired well driller drop-down is not displayed or does not have a value selected.
Exp. Driller: (Conditional) The name and expired certification number of the well driller who was likely to drill the well(s) listed in the permit. This applies if the well driller had a certification number that is now expired at the time when the permit was issued. This only displays if it has a value, and is required if a certified well driller is not selected.
Role: (Required) The applicant’s role which can be either Certified Driller, Owner or Other.
Applicant First Name: (Conditional) The first name of the applicant. If a company name is not entered then the first or middle name is required.
Applicant Middle Name: (Conditional) The middle name of the applicant. If a company name is not entered then the first or middle name is required.
Applicant Last Name: (Conditional) The last name of the applicant. If a company name is not entered then the last name is required.
Applicant Company: (Conditional) The company that applied for the permit. If a first or middle and last name are not entered then the company is required.
Applicant Street: (Required) The applicant's street address.
Applicant City: (Required) The applicant's city.
Applicant State: (Required) The applicant's state.
Applicant Zip: (Required) The applicant's zip code.
Applicant Phone: (Required)The applicant's phone number.
Property Owner First Name: (Conditional) The first name of the property owner. If the applicant is not the owner and a company name is not entered then the first or middle name is required.
Property Owner Middle Name: (Conditional) The middle name of the property owner. If the applicant is not the owner and a company name is not entered then the first or middle name is required.
Property Owner Last Name: (Conditional) The last name of the property owner. If the applicant is not the owner and a company name is not entered then the last name is required.
Property Owner Company: (Conditional) The company that owns the well. If the applicant is not the owner and a first or middle and last name are not entered then the company is required.
Property Owner Street: The property owner's street address.
Property Owner City: The property owner's city.
Property Owner State: The property owner's state.
Property Owner Zip: The property owner's zip code.
Property Owner Phone: The property owner's phone number.
Well Number: The unique number generated by the system for identifying each well. Clicking on the well number takes the user to the well's details.
Permit Well Status: The status of the permit for a well, which can be Active, Completed, Log Completed, Water Test Completed, Expired or Withdrawn.
E911 House Number: The house number of the E911 address of the well.
E911 Street: The street address of the E911 address of the well.
E911 City: The city of the E911 address of the well.
Parcel Number: The value used at the county level for identifying a well location.
Construction Date: (Required) The anticipated construction date of the proposed well.
Depth: (Required) The estimated depth of the proposed well.
Map Type: (Required) The method in which the latitude and longitude coordinates were found.
Latitude: (Required) The user can either enter the latitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
Longitude: (Required) The user can either enter the longitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
PLS: This information is read-only. This information will automatically be populated when the user returns from the Map screen. If PLS information is based on estimated section-corners, “(estimated)” will appear after the PLS description.
Purpose: (Required) The use(s) of the well.
Variance Requested: Any variances that have been approved for the permit.
Special Conditions: Any special conditions that have been approved for the permit.
 
Buttons:
Search Wells: Only displays when adding an existing well. Searches for a well log matching the well number entered into the text box. If the well number is found then the well information is displayed. If it is not currently tied to a permit and is the same type as the current permit (Heat Pump, Monitoring Well or Water Supply Well and Other Uses), then the "Add Well to Permit" link will be displayed, otherwise an error message will be displayed.
Cancel: Only displays when adding an existing well. Cancels the adding of an existing well.
Get Map...: Opens the map window. In the map window the user can navigate to the location of the proposed well and capture the center point of the map to get the latitude and longitude.
Save: Saves the permit. If the user does not have permission to edit the permit then the button will be disabled.
Cancel: Returns the user to the Permit Search screen.
Delete: Deletes the permit. This only displays when editing an existing permit. If the user does not have permission to delete the permit then the button will be disabled.
 
Links:
Add New Well: Adds a new set of blank well fields to the permit. After the required fields are filled in and the permit is saved the proposed well will be added to the permit.
Add Existing Well: Displays a text box for entering a well number, a search button for searching for the well number and a cancel button.
Add Well to Permit: Ties an existing well to the permit. This only displays when adding an existing well.
Well Number: Takes the user to the well's details.
Update Status: Updates the permit status for a proposed well. This only displays when the user is a sanitarian that does not have permission to edit the rest of the permit.

Well Log Search



Description:
Search for an existing well or heat pump log. The user can get the well or heat pump log information by entering the appropriate search criteria, and clicking on the search button.
 
Fields:
Certified Well Driller: The name and current certification number of the well driller who drilled the well or heat pump.
Expired Well Driller: The name and expired certification number of the well driller who drilled the well or heat pump. This applies if the well driller had a certification number that is now expired at the time when the well was drilled.
Well Number: The unique number generated by the system for identifying each well or heat pump.
Drill Date From: The well or heat pump drill completion date (beginning date range).
Drill Date To: The well or heat pump drill completion date (ending date range).
County: The name of the county where the well or heat pump is located.
Geosam Import: Wells that were transferred from the Iowa Geological Survey.
First Name: The property owner’s first name.
Middle Name: The property owner’s middle name.
Last Name: The property owner’s last name.
Company: The name of the company that owns the property.
First Name: The well user’s first name.
Middle Name: The well user’s middle name.
Last Name: The well user’s last name.
Company: The name of the company that uses the well.
Quarter 5: The quarter of the quarter of the quarter of the quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 4: The quarter of the quarter of the quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 3: The quarter of the quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 2: The quarter of the quarter of the section, township, range of the well or heat pump.
Quarter 1: The quarter of the section, township, range of the well or heat pump.
Section: The section of the well or heat pump location.
Township: The township of the well or heat pump location.
Range: The range of the well or heat pump location.

Description:
Displays a list of wells or heat pumps matching the search criteria entered by the user in the Well Log Search screen. The user can get the details of a particular well by clicking on the well number.

Well Number: The unique number generated by the system for identifying each well or heat pump. Clicking on the well number takes the user to the well or heat pump's details.
County: The name of the county where the well or heat pump is located.
Property Owner: The name of the person or company that owns the property.

Description:

Geosam Reviewed: Indicates if the Iowa Geological Survey has reviewed the well record.

Property Owner Middle Name: (Conditional) The middle name of the property owner. If a company name is not entered then the first or middle name is required.
Property Owner Last Name: (Conditional) The last name of the property owner. If a company name is not entered then the last name is required.
Property Owner Company: (Conditional) The company that owns the well. If a first or middle and last name are not entered then the company is required.
Property Owner Street: (Required) The property owner's street address.
Property Owner City: (Required) The property owner's city.
Property Owner State: (Required) The property owner's state.
Property Owner Zip: (Required) The property owner's zip code.
Property Owner Phone: The property owner's phone number.
Map Type: (Required) The method in which the latitude and longitude coordinates were found.
Latitude: (Required) The user can either enter the latitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.

Date Completed Confidence: (Required) The confidence level that the date completed is accurate.
Samples have been submitted to IGS for analysis: Indicates whether or not water samples have been submitted to the Iowa Geological Survey for analysis.
Well Depth: (Required) The depth of the well.
Well Depth Confidence: (Required) The confidence level that the well depth is accurate.
Depth to Water: The depth to the surface of the water.
Topography: The topographical position of the well on the landscape.
Elevation: The relative elevation, compared to sea level, of the wellhead.
Construction Method: (Required) The type of well construction equipment used to construct the well.
Data Entered By: The name of the person entering the well details (defaults to the current user's username).
Well Driller: (Required) The name of the well driller who drilled the well. Along with the well driller’s name, the certification number is also displayed. If the well driller had a certification that is now expired an additional drop-down will be displayed for expired well drillers.
Contract Company: The name of the contractor company the well driller is associated with. Along with the contractor company name, the city in which it resides is also displayed.
Well Use: (Required) The use(s) of the well.
Descriptor: (Conditional) The value used to identify a monitoring well. This is only required for monitoring wells.
Number of Wells: (Conditional) The number of heat pumps drilled under the associated permit. This is only required for heat pumps.
Well Was: (Required) The method used to develop the well. This field is not required when editing older well logs.
For: (Required) The length of time the well was pumped or developed. This field is not required when editing older well logs.
At: (Required) The pumping rate used to develop the well in gallons per minute. This field is not required when editing older well logs.
Explain: Further explaination of the well development.
Pump Installed: Indicates whether or not the pump was installed.
Date: (Conditional) The date that the pump was installed. This is required if the pump has been installed.
Type of Pump: (Conditional) The type of pump installed. This is required if the pump has been installed.
Pump Diameter: (Conditional) The diameter of the pump installed. This is required if the pump has been installed.
Installer's Name (text box): The name of the pump installer who installed the pump. This field is for older well logs which had the pump installer's name typed in.
Installer's Name (drop-down): (Conditional) The name of the pump installer who installed the pump. Along with the pump installer’s name, the certification number is also displayed. This is required if the pump has been installed.
Depth to Intake: (Conditional) The depth from the ground surface to the level of the pump intake. This is required if the pump has been installed.
Rated Capacity: (Conditional) The capacity of the well pump in gallons per minute. This is required if the pump has been installed.
From: (Required) The depth of the top of this segment of the well.
To: (Required) The depth of the bottom of this segment of the well.
Lithology: (Required) A description of the material in this segment of the subsurface formation.
Color: (Required) The color of the soil.
Comments: Any other useful information.
Remarks: Such as depth of lost circulation, materials or tools.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Hole Size: (Required) The diameter of the hole constructed in the ground.
Comments: Any other useful information.
Casing Type: (Required) The material used to construct the casing for this segment of the hole.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Diameter: (Required) The diameter of the casing for this segment of the hole.
ID/OD: (Required) Indicates whether the diameter specified is the inside or outside diameter.
STD/Thickness: (Required) The casing weight per foot for steel casing or wall thickness for plastic casing.
Comments: Any other useful information.
Grouting Type: (Required) The grout material used.
Grouting Method: (Required) The method used for grouting.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Amount: (Required) The amount of grout used.
Amount Unit: The measurement unit of the amount of grout.
Comments: Any other useful information.
Material: (Required) The well screen material such as stainless steel or plastic.
Diameter: (Required) The diameter of the well screen.
Slot Size: (Required) The well screen slot size in thousandths of an inch.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Comments: Any other useful information.
Bottom Capped: Indicates whether or not a cap has been placed on the bottom of the casing.
Capping Material: (Conditional) The type of bottom cap that was used. This is required if a cap has been placed on the bottom of the casing.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Gravel Type: (Required) The material used for gravel pack.
Weight: (Required) The weight of the gravel pack in pounds.
Comments: Any other useful information.
Seals Type: The type of seal used.
Seals Depth: The depth that the seal was placed.
Packers Type: The type of packer used.
Packers Depth: The depth that the packer was placed.
Aquifer: (Required) The type of material that constitutes the primary water bearing aquifer of this well. This field is not required when editing older well logs.
Main Water-Supply Zone from: (Required) The depth of the top of the primary aquifer supplying this well. This field is not required when editing older well logs.
To: (Required) The depth of the bottom of the primary aquifer supplying this well. This field is not required when editing older well logs.
Static Water Level: (Required) The static water level of the well. This field is not required when editing older well logs.
Above/Below GL: (Required) Indicates if the static water level is above (artesian) or below the ground level. This field is not required when editing older well logs.
Measurement Method: (Required) How the static water level was measured. This field is not required when editing older well logs.
Pumping Water Level: (Required) The pumping water level or static level plus drawdown. This field is not required when editing older well logs.
Measurement Method: (Required) How the pumping water level was measured. This field is not required when editing older well logs.
A Yield of: (Required) The estimated yield of the well. This field is not required when editing older well logs.
Measurement Method: (Required) How the yield was measured. This field is not required when editing older well logs.
Measurements Taken at: (Required) The hour, minute and date that the yield measurement was taken. This field is not required when editing older well logs.
 
Buttons:
Get Map...: Opens the map window. In the map window the user can navigate to the location of the well and capture the center point of the map to get the latitude and longitude.
Save: Saves the well log. If the user does not have permission to edit the well then the button will be disabled.
Cancel: Returns the user to the Well Log Search screen.

Add Other Well User: Takes the user to the Well User Information screen, where they enter the new other well user's information
Edit: Takes the user to the other well user's details.
Update Owner Information: Takes the user to the Owner Information screen, where they enter the previous owner's information
Edit: Takes the user to the previous owner's details.
Set Current Owner: Sets a previous owner to the current owner, and adds the former current owner to the previous owners.
Add Rows: Adds new rows to the grids for well formation log, hole size, casing, grout, well screen and gravel pack. Enter the number of rows to be added in the textbox or one row will be added by default. Entering a negative number will remove blank rows.
Add Renovation: Takes the user to the Renovation screen, where they enter the new renovation. If the well is plugged then this link will be disabled.
Edit/View: Takes the user to the renovation details. If the user has permission to edit the details then "Edit" will be displayed, otherwise "View" will be displayed.
Add Water Test: Takes the user to the Water Test screen, where they enter the new water test. If the well is plugged then this link will be disabled.
Edit/View: Takes the user to the water test details. If the user has permission to edit the details then "Edit" will be displayed, otherwise "View" will be displayed.
Add Plug Information: Takes the user to the Plug Information screen, where they enter the new plug information.
Edit/View: Takes the user to the plug information details. If the user has permission to edit the details then "Edit" will be displayed, otherwise "View" will be displayed.

Well User Information



Description:
Allows the user to enter new well users or edit existing well users.
 
Fields:
First Name: (Conditional) The first name of the well user. If a company name is not entered then the first or middle name is required.
Middle Name: (Conditional) The middle name of the well user. If a company name is not entered then the first or middle name is required.
Last Name: (Conditional) The last name of the well user. If a company name is not entered then the last name is required.
Company: (Conditional) The company that uses the well. If a first or middle and last name are not entered then the company is required.
Street: (Required) The well user's street address.
City: (Required) The well user's city.
State: (Required) The well user's state.
Zip: (Required) The well user's zip code.
Phone: The well user's phone number.
 
Buttons:
Save: Creates a new well user or edits an existing well user.
Cancel: Returns the user to the Well Details screen.
Delete: Deletes the well user. This only displays when editing an existing well user.

Owner Information



Description:
Allows the user to enter new previous owners or edit existing previous owners.
 
Fields:
First Name: (Conditional) The first name of the owner. If a company name is not entered then the first or middle name is required.
Middle Name: (Conditional) The middle name of the owner. If a company name is not entered then the first or middle name is required.
Last Name: (Conditional) The last name of the owner. If a company name is not entered then the last name is required.
Company: (Conditional) The company that owns/owned the well. If a first or middle and last name are not entered then the company is required.
Street: (Required) The owner's street address.
City: (Required) The owner's city.
State: (Required) The owner's state.
Zip: (Required) The owner's zip code.
Phone: The owner's phone number.
Set Current Owner: Only displays when entering a new owner. Indicates if the new owner is the current owner.
Ending Date: (Conditional) The date that ownership of the property ended. This is required if they are not the current owner. If they are the current owner then this field will be hidden.
 
Buttons:
Save: Creates a new owner or edits an existing owner.
Cancel: Returns the user to the Well Details screen.

Renovation



Description:
Allows the user to create, edit and delete renovations depending on their privileges.
 
Fields:
Well Number: The unique number generated by the system for identifying each well.
County: The name of the county where the well is located.
Eligible for Grants To County Program: (Required) Indicates whether or not the renovation is eligible for the Grants To County program.
Amount paid by county for renovation: (Conditional) Amount paid by the county towards the cost of the renovation. This is required if the renovation is eligible for the Grants To County program.
Renovation Date: (Required) The date that the well was renovated.
Well Depth: The depth of the well after the renovation.
Depth of Water: The depth of the water after the renovation.
Casing Diameter: The diameter of the casing after the renovation.
Casing Depth: The depth of the casing after the renovation.
Renovated Casing Type: The material that the casing is made of.
Construction Method: The well construction technique for the original well.
Brief description of the work: (Required) A brief description of the work done in renovating the well.
Well Driller: (Required) The well driller who renovated the well. The well driller's name and certification number are displayed. If the well driller had a certification that is now expired an additional drop-down will be displayed for expired well drillers.
County Agent: (Conditional) The name of the county agent approving the well renovation. This is required if the renovation is eligible for the Grants To County program.
Current Property Owner Information: This information is read-only. These fields display contact information for the person or company who owned the property at the time of the renovation.
 
Buttons:
Save: Saves the renovation. If the user does not have permission to edit the renovation then the button will be disabled.
Cancel: Returns the user to the Well Details screen.
Delete: Deletes the renovation. This only displays when editing an existing renovation. If the user does not have permission to delete the renovation then the button will be disabled.

Water Test



Description:
Allows the user to create, edit and delete water tests depending on their privileges.
 
Fields:
Well Number: The unique number generated by the system for identifying each well.
County: The name of the county where the well is located.
Entry Date: The date that the water test was entered into the system.
Eligible for Grants To County Program: (Required) Indicates whether or not the water test is eligible for the Grants To County program.
County Agent: (Required) The name of the county agent approving the well water test.
Water Test Location: (Required) The address where the water test sample was taken.
Is wellhead sealed?: (Required) Is the top of the well casing sealed to prevent insect and rodent entry?
Is wellhead covered?: (Required) Is the top of the well casing even covered or completely open?
Is wellhead in pit?: (Required) Does the top of the well casing end inside and near the bottom of a pit?
Is visible casing intact?: (Required) Is the portion of the well casing that you can see appear to be in good condition of is it badly deteriorated?
Is casing >1' above grade?: (Required) Does the top of the well casing terminate at least one foot above the ground surface?
Is Cistern in use?: (Required) Does the water system incorporate a cistern for storage of pumped water before entering the house?
Describe other adverse conditions: Are there any other conditions of the well that could contribute to aquifer contamination?
<50' from septic tank?: (Required) Is the well less than 50 feet from the house septic tank?
<100' from absorption field?: (Required) Is the well less than 100 feet from the wastewater soil absorption system?
<100' from any livestock?: (Required) Is the well less than 100 feet from any livestock fedding facility?
<100' from any fuel tanks?: (Required) Is the well less than 100 feet from any above ground or buried liquid fuel tank?
<300' from chemical storage?: (Required) Is the well less than 300 feet from nay hazardous chemical storage?
<100' from Abandoned Well?: (Required) Is the well less than 100 feet from any old abandoned well?
Describe other potential contaminants: Is the well too close to any other possible hazardous contamination source.
List water treatment system used: The type of water treatment system listed used.
Where was the sample taken?: (Required) The place from where the sample was taken.
Before/After Treatment?: (Required) Indicates whether the sample was taken before or after the water treatment.
Mention any historical contamination of which the owners are aware: Were there any spills or known contamination of this well historically?
Date Sampled: (Required) The date that the water sample was collected for testing.
Sample Collector: (Required) The person collecting the sample.
Laboratory: (Required) The name of the laboratory where the water sample was tested.
Coliform Tested: (Required) Indicates whether or not the coliform test was conducted.
If 'Yes' provide details: (Conditional) Indicates whether the coliform was present or absent. This is required if the coliform test was conducted.
Coliform Measurement Method: This field is read-only. It is dependent on the laboratory that conducted the test.
Coliform Level: The level of coliform present in the water sample if known.
Fecal/E. Coli Coliform Tested: (Required) Indicates whether or not the Fecal coliform test was conducted.
If 'Yes' provide details: (Conditional) Indicates whether the fecal coliform was present or absent. This is required if the fecal coliform test was conducted.
Fecal/E. Coli Coliform Measurement Method: This field is read-only. It is dependent on the laboratory that conducted the test.
Fecal/E. Coli Coliform Level: The level of fecal coliform present in the water sample if known.
Nitrate Tested: (Required) Indicates whether or not the nitrate test was conducted.
If 'Yes' provide details: (Conditional) Which unit was used for expressing the nitrate concentration? Type can be either as Nitrate or as Nitrogen. This is required if the nitrate test was conducted.
Nitrate Measurement Method: This field is read-only. It is dependent on the laboratory that conducted the test.
Nitrate Level: (Conditional) The level of nitrate present in the water sample. This is required if the nitrate test was conducted.
Mention if there are any other constituents: Mention if any other constituents were tested for and to what levels were they detected.
 
Buttons:
Save: Saves the water test. If the user does not have permission to edit the water test then the button will be disabled.
Cancel: Returns the user to the Well Details screen.
Delete: Deletes the water test. This only displays when editing an existing water test. If the user does not have permission to delete the water test then the button will be disabled.

Plug Information



Description:
Allows the user to create, edit and delete plug information depending on their privileges.
 
Fields:

Amount paid by county for plugging: (Conditional) Amount paid by the county towards the cost of the plugging. This is required if the plugging is eligible for the Grants To County program.
Plug Date: (Required) The date that the well was plugged.
Describe the well location: (Required) A brief description of the well location.
Well Driller: (Required) The well driller who plugged the well. The well driller's name and certification number are displayed. If the well driller had a certification that is now expired an additional drop-down will be displayed for expired well drillers.

Heat Pump Details



Description:
This purpose of this screen is to maintain heat pump information. The user can create, view, update or delete heat pump information depending on their privileges.
 
Fields:
Permit Number: The unique number generated by the system for identifying each permit.
Heat Pump Number: The unique number generated by the system for identifying each heat pump.
County: (Required) The name of the county where the heat pump is located.
Heat Pump Status: (Required) The status of the heat pump, which can be Permitted, Active, Active Logged, Active Water Test, Standby, Inactive, Retired, Plugged or Withdrawn.
Geosam Reviewed: Indicates if the Iowa Geological Survey has reviewed the heat pump record.
Property Owner First Name: (Conditional) The first name of the property owner. If a company name is not entered then the first or middle name is required.
Property Owner Middle Name: (Conditional) The middle name of the property owner. If a company name is not entered then the first or middle name is required.
Property Owner Last Name: (Conditional) The last name of the property owner. If a company name is not entered then the last name is required.
Property Owner Company: (Conditional) The company that owns the heat pump. If a first or middle and last name are not entered then the company is required.
Property Owner Street: (Required) The property owner's street address.
Property Owner City: (Required) The property owner's city.
Property Owner State: (Required) The property owner's state.
Property Owner Zip: (Required) The property owner's zip code.
Property Owner Phone: The property owner's phone number.
Map Type: (Required) The method in which the latitude and longitude coordinates were found.
Latitude: (Required) The user can either enter the latitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
Longitude: (Required) The user can either enter the longitude manually or use the Get Map button to obtain it. If values are entered for latitude and longitude then the map will center on that location.
PLS: This information is read-only. This information will automatically be populated when the user returns from the Map screen. If PLS information is based on estimated section-corners, “(estimated)” will appear after the PLS description.
E911 House Number: The house number of the E911 address of the heat pump.
E911 Street: The street address of the E911 address of the heat pump.
E911 City: The city of the E911 address of the heat pump.
Date Completed: (Required) The date that heat pump drilling was completed.
Date Completed Confidence: (Required) The confidence level that the date completed is accurate.
Bore Hole Depth: (Required) The depth of the bore hole.
Number of Loops: (Required) The number of bore holes per heat pump system.
Loop Diameter: (Required) The diameter of the loop pipe.
Loop Length(s): (Required) The depth of the loop in the bore hole.
Loop Pipe Manufacturer: (Required) The manufacturer and the weight of the loop pipe.
Driller: (Required) The name of the driller who drilled the heat pump. Along with the driller’s name, the certification number is also displayed.
Data Entered By: The name of the person entering the heat pump details (defaults to the current user's username).
Comments: Any additional information.
From: (Required) The depth of the top of this segment of the well.
To: (Required) The depth of the bottom of this segment of the well.
Lithology: (Required) A description of the material in this segment of the subsurface formation.
Color: (Required) The color of the soil.
Comments: Any other useful information.
Remarks: Such as depth of lost circulation, materials or tools.
Grouting Type: (Required) The grout material used.
Grouting Method: (Required) The method used for grouting.
Top: (Required) The depth of the top of this segment of the well.
Bottom: (Required) The depth of the bottom of this segment of the well.
Amount: (Required) The amount of grout used.
Amount Unit: The measurement unit of the amount of grout.
Comments: Any other useful information.
 
Buttons:
Get Map...: Opens the map window. In the map window the user can navigate to the location of the heat pump and capture the center point of the map to get the latitude and longitude.
Save: Saves the heat pump. If the user does not have permission to edit the heat pump then the button will be disabled.
Cancel: Returns the user to the Well Log Search screen.
Delete: Deletes the heat pump. This only displays when editing an existing heat pump. If the user does not have permission to delete the heat pump then the button will be disabled.

Add Other Well User: Takes the user to the Well User Information screen, where they enter the new other well user's information
Edit: Takes the user to the other well user's details.
Update Owner Information: Takes the user to the Owner Information screen, where they enter the new previous owner's information

Set Current Owner: Sets a previous owner to the current owner, and adds the former current owner to the previous owners.
Add Rows: Adds new rows to the formation and grout grids. Enter the number of rows to be added in the textbox or one row will be added by default. Entering a negative number will remove blank rows.
Add Plug Information: Takes the user to the Plug Information screen, where they enter the new plug information.
Edit/View: Takes the user to the plug information details. If the user has permission to edit the details then "Edit" will be displayed, otherwise "View" will be displayed.

Grants To Counties - New Fiscal Year Setup



Description:
This screen helps the business leader in managing ‘Setting up of the new fiscal year’ as part of the Grants to Counties program. The Business Leader needs to enter the annual budget and select a list of the non-participate counties (if any) for the Grants to Counties program for the next fiscal year.
 
Fields:
Amount: (Required) The amount allocated for the Grants To Counties program.
Participating Counties: Counties participating in the Grants To Counties program.
Non Participating Counties: Counties not participating in the Grants To Counties program.
 
Buttons:
Next: Takes the user to the New Fiscal Year Overview screen which displays the calculations in setting up the new fiscal year.
Cancel: Cancels out of the GTC - Set Up New Fiscal Year process and returns the user to the home page.

Grants To Counties - New Fiscal Year Overview



Description:
This screen displays the calculations in setting up the new fiscal year program. It will show all the counties participating in the Grants to Counties program along with the allocated, reserved, leftover and payable amounts for each county. The Business Leader can either save the information if they are satisfied with the calculations or if for some reason the Business Leader is not satisfied with the calculations in setting up the new fiscal year, they may decline the calculations in which case the system is restored to its original state prior to executing the process of setting up new fiscal year.
 
Fields:
County: The name of the county.
Allocated Amount: The amount allocated to a county for the current fiscal year.
Reserve Amount: The Allocated amount minus the payable amount for a county.
Leftover Amount: The unused paid amount in the previous fiscal year carried over the current fiscal year for a county.
Payable Amount: The amount marked as paid for the current fiscal year for a county (25% of the Allocated amount - leftover amount).
 
Buttons:
Submit: Saves the information to the database.
Return: Takes the user back to the New Fiscal Year Setup screen.

Grants To Counties - Periodic Payout Setup



Description:
This screen helps the business leader in making Periodic Payments under the Grants to Counties program. The termination date is defaulted to the current date; the user may change it if required.
 
Fields:
Termination Date: (Required) The cut off date for calculating the periodic payout.
Final Payment for the Fiscal Year: Checking this field enables the user to make payments less than $1000.
 
Buttons:
Make Periodic Payout: Takes the user to the Periodic Payout Overview screen which displays periodic payout calculations.

Description:
This screen shows the estimates in making the periodic payments. It will show all the counties participating in the Grants to counties program along with wells tested, renovated and plugged and the associated costs for each county. The Business Leader can either save the information if they are satisfied with the calculations or they may decline the calculations, if for some reason the Business Leader is not satisfied with the calculations in making the periodic payments in which case the system is restored to its original state prior to executing the process of making periodic payments.
 
Fields:
County: The name of the county.
Wells Tested: The total number of wells tested under the GTC program for a county.
Water Test Cost Incurred: The cost incurred in testing the wells under the GTC program for a county.
Wells Renovated: The total number of wells renovated under the GTC program for a county.
Renovation Cost Incurred: The cost incurred in renovating the wells under the GTC program for a county.
Wells Plugged: The total number of wells plugged under the GTC program for a county.
Well Plug Cost Incurred: The cost incurred in plugging the wells under the GTC program for a county.
Total Cost Incurred: The total cost incurred in testing, renovating and plugging wells under the GTC program for a county.
Payable Amount: The amount paid to a county under the ‘Periodic Payout’ process as part of the GTC program for a county.
 
Buttons:
Submit: Saves the information to the database.
Return: Takes the user back to the Periodic Payout Setup screen.

Report Viewer



Description:
Displays reports and allows users to export them into various formats. Also provides functionality for navigation, zooming, searching for text, refreshing the data, and printing.
 
Fields:
Page Number: A text box for entering the page of the report. The user enters a valid page number and hits the Enter button to go to that page.
Zoom Level: A drop-down list for choosing the zoom level of the report. The default zoom level is 100%.
Search Text: A text box for entering text to search for. The user enters the text and hits the Enter button or clicks the "Find" link to go to the first occurrence of that text. Clicking the "Next" link will take the user to the next occurrence.
Export Format: A drop-down list for choosing the export format of the report. After selecting the format the user clicks on the "Export" link.
 
Buttons:
First Page Navigates to the first page of the report.
Previous Page Navigates to the previous page of the report.
Next Page Navigates to the next page of the report.
Last Page Navigates to the last page of the report.
Refresh Refreshes the data in the report.
Print Prints the report. The user may be prompted to install software the first time.
 
Links:
Find: Finds the first occurrence of the search text in the report.
Next: Finds the next occurrence of the search text in the report.
Export: Exports the report into the selected format.
Permit Number: Opens the permit details in a new window.
Well Number: Opens the well details in a new window.
 
Report Descriptions:
Permits: Displays a list of permits that have been issued for the given criteria. Allows the user to filter on date, well driller, permit status and county. Includes information about the permit such as county, permit number, issue date, owner, driller, agent and permit status. This report is available to sanitarians and administrators.
Overdue Logs: Displays a list of wells that have been in an Active or Active Water Test state greater than or equal to the specified number of days for the given county. Includes information about the well such as the county, permit issue date, permit number, number of days in it's current state, well number, owner and well driller. These absent well logs are the underlying cause of the permit to remain active. This report is available to sanitarians and administrators.
Expired Permits: Displays a list of permits that have been in an Expired state greater than or equal to the specified number of days for the given county. Includes the county the permit was issued in, permit issue date, permit number, well number, well status and owner. This report is available to sanitarians and administrators.
Permit Printout: Generates a permit application form. This is available to the general public.
Payment Voucher: Generates a payment voucher for unpaid permits for the given county and date range specified. Permits that fit the criteria are displayed in a grid prior to the generation of the payment voucher. The grid includes columns for permit number, date requested, date approved and the permit's payment status. Administrators can toggle the permit between paid and unpaid. Administrators and sanitarians can generate payment vouchers.
Overdue Water Tests: Displays a list of wells that have been drilled greater than or equal to a specified number of days or drill date and do not have a corresponding water record entered into the system for the given county. Includes the county, well number, drill date, the number of days since drilled, owner and site address. This report is available to sanitarians and administrators.
Bad Water Tests: Displays a list of water tests that reported unacceptable bacteria or nitrate levels for the given criteria. Allows the user to filter based on location, county, fail type, well depth, use, casing type and test date. Includes the well number, test date, location, bacteria and nitrate levels, well depth and county. Bacteria tests fail when bacteria levels are found present in a water test. Nitrate failure occurs when the amount of Nitrate (as Nitrogen) present is above 10. This report is available to the general public.
Water Tests Detail: Displays a list of well water tests for the given county and date range. Includes the well number, county, owner, date tested, pass/fail status for bacteria and nitrate tests, location and whether or not it is eligible for a grant. Totals for the number of grants and passed tests are also given. This report is available to the general public.
Water Test Form: Generates a standard water test form. This is available to sanitarians and administrators.
Well Drillers: Displays a current list of certified well drillers and their corresponding contact information. The user can sort by name or certification ID. This report is available to the general public.
Pump Installers: Displays a current list of certified pump installers and their corresponding contact information. The user can sort by name or certification ID. This report is available to the general public.
Well Pluggers: Displays a current list of certified well pluggers and their corresponding contact information. The user can sort by name or certification ID. This report is available to the general public.
Drilled Wells: Displays a list of wells that have completed the drill process for the given company, well driller, drill date range and county. Displays the well number, drill date, owner, driller, contractor, well status, and county. This report is available to sanitarians and administrators.
Inactive Wells: Displays a list of wells that are in an Inactive state for the given county. Includes the county, well number, owner, and well location. This report is available to sanitarians and administrators.
Well Printout: Displays a summary of a well record for a given well number. Includes information on the owner, well depth, date completed, location, topography, elevation, formation log, development, pump, uses, contractor, driller, drill method, hole size, casing, grout, screen and water levels. This report is available to the general public.
Plugged Wells Detail: Displays a list of wells that have been plugged for a given county and date range. Includes the well number, county, owner, date plugged, whether or not it received a grant and the location. A total for the number of wells plugged with grants is also given. This report is available to the general public.
Renovated Wells Detail: Displays a list of wells that have been renovated for a given county and date range. Includes the well number, county, owner, date renovated, whether or not it received a grant and the location. A total for the number of wells renovated with grants is also given. This report is available to the general public.
Well Depth: Displays a list of wells for a given section, township, range and direction. Includes the well number, owner name and location, well depth, well status, last water test and casing type. This report is available to the general public.
Grants To Counties: Displays a summary of Grants to Counties information for all participating counties. Includes the amount of money that a participating county has earned compared with how much they have been paid for renovations, well plugs, and water tests. Also displays each county’s maximum allocated amount for the year. This report is available to the general public.
County Data Export: Allows the user to export well data into an XML format for a given county and date range. Sanitarians can only export data for their own counties. Administrators can export data for all counties.
